摘要

目的 探讨基于物联网的脉冲磁疗联合运动疗法的远程康复新模式对膝骨关节炎(knee osteoarthritis,KOA)患者的临床疗效.方法 选取 2022 年 10 月至 2023 年 3 月,于我科就诊的 43 例 KOA 患者,其中男 13 例,女 30 例;年龄 42~80 岁,平均(62.66±11.97)岁.进行为期 4 周的远程康复治疗,包括基于物联网的脉冲磁疗联合运动疗法指导.所有受试者分别在治疗前、治疗后 4 周评估,主要结果使用西安大略和麦克马斯特大学骨关节炎指数(Western Ontario and McMaster Universities Osteoarthritis index,WOMAC)评分量表.次要结果指标包括:疼痛数字评分量表(numerical rating scale,NRS)、生活质量调查表(short form-36,SF-36)、日常生活能力(instrumental activity of daily living,IADL)、焦虑自评量表(self-rating anxiety,SAS)、抑郁自评量表(self-rating depression,SDS)、起立行走试验(timed"up&go",TUG),并在治疗后记录患者对治疗的满意度和不良反应.结果 所有患者远程康复后 4 周,WOMAC 总分、疼痛、僵硬及功能评分较治疗前均降低,疼痛从治疗前 13.85±9.09 降至 9.69±8.63,僵硬从治疗前 5.04±3.14 降至 4.46±3.78(P 均<0.05).NRS 从治疗前 4.40±2.52 降至 2.42±2.30,SAS 从治疗前 40.62±8.89 降至 35.88±7.55,SDS 从治疗前46.18±11.94 降至 39.05±8.37(P 均<0.05),TUG 从治疗前 10.73±6.10 缩短至 8.88±3.66(P<0.05),SF-36从治疗前 107.61±20.62 增加至 115.01±16.06(P<0.05).所有患者未出现不良反应,患者满意度为 100%.结论 基于物联网的脉冲磁疗联合运动疗法的远程康复治疗显著改善 KOA 患者的膝关节疼痛和僵硬,焦虑及抑郁心理状态改善明显,显著提高步行能力及生活质量.

Abstract

Objective To explore the clinical effectiveness of Internet of Things-based(Iot-based)pulsed electromagnetic fields combined exercise therapy in patients with knee osteoarthritis.Methods From October 2022 to March 2023,43 patients with knee osteoarthritis(KOA)aged≥40 years were analyzed,including 13 males and 30 females.The average age was(62.66±11.97)years,ranging from 42 to 80 years.Therapy of four weeks was applied,including Iot-based pulsed electromagnetic fields combined exercise therapy.All subjects were evaluated before and 4 weeks after treatment.Primary outcome index:Western Ontario and McMaster Universities Osteoarthritis(WOMAC)scale.Secondary outcomes:Pain Numerical Rating Scale,Quality of Life Questionnaire(Short Form-36,SF-36),Instrumental Activity of Daily Living(IADL),Self-Rating Anxiety(SAS),Self-Rating Depression(SDS),Timed"Up&Go"(TUG),Patients'satisfaction with the treatment and adverse reactions.Results After 4 weeks of tele-rehabilitation,the total WOMAC score,pain,stiffness and functional scores of all patients were decreased compared with those before treatment.After 4 weeks of tele-rehabilitation,pain decreased from 13.85±9.09 to 9.69±8.63(P<0.05),stiffness decreased from 5.04±3.14 to 4.46±3.78(P<0.05),NRS decreased from 4.40±2.52 to 2.42±2.30(P<0.05),SAS decreased from 40.62±8.89 to 35.88±7.55(P<0.05),SDS decreased from 46.18±11.94 to 39.05±8.37(P<0.05),TUG decreased from 10.73±6.10 to 8.88±3.66(P<0.05),and SF-36 increased from 107.61±20.62 to 115.01±16.06(P<0.05).No adverse reactions occurred in all patients,and patient satisfaction was 100%.Conclusions Iot-based pulsed electromagnetic fields combined exercise therapy can significantly reduce knee pain,stiffness,anxiety and depression in patients with KOA,and significantly improve walking ability and quality of life.
