Clinical analysis of 31 cases with soft tissue recurrence of giant cell tumor of bone
Objective To investigate the soft tissue recurrence of giant cell tumor of bone(GCTB),and thus improving the understanding of soft tissue recurrence after surgical treatment of GCTB.Methods From January 2003 to December 2020,patients pathologically confirmed recurrence of GCTB in Department of Orthopaedic Oncology Surgery,Beijing Jishuitan Hospital were screened.Patients with soft tissue recurrence were selected. The age, gender, original lesion site,Campanacci grade, time to soft tissue recurrence after primary surgery, recurrence times,follow-up time,MSTS score and complications were record. Results A total of 31 patients with soft tissue recurrence after GCTB surgery were included(male/female:10:21).Soft tissue recurrence rate in patients with recurrence of GCTB was 5.73%.The mean age was(28.52±9.93)years(range:16-57 years). The original GCTB located in the proximal end of the fibula and distal end of the ulna in 4 cases, respectively;proximal femur and proximal tibia in 3 cases, respectively;distal end of the femur in 6 cases;distal end of the radius in 8 cases;the first metacarpal bone, the first metatarsal bone and the patella in 1 case, respectively.Campanacci grade at the time of initial operation:2 cases of grade Ⅰ,9 cases of grade Ⅱ,and 20 cases of grade Ⅲ.The mean time from surgery to soft tissue recurrence was(23.23±26.12)months(range:2-127 months). After the primary operation, recurrence occurred once in 12 cases, twice in 12 cases and three times in 7 cases.Patients recurred for two or more times accounted for 61.3%.Multiple metastasis, death after lung metastasis and hip amputation occurred in one case, respectively. The final MSTS score was(26.62±4.21)(range:14-30).Conclusions Although soft tissue recurrence of GCTB is rare,it is prone to multiple recurrence and may lead to amputation, death and other serious consequences, which should be paid attention to in clinic.
