Objective To investigate the perforation status in children with acute appendicitis and analyze the independent risk factors.Methods The case data of 182 children with acute appendicitis were retrospectively analyzed.According to the pathological result of postoperative appendix perforation or not,they were divided into perforation group(n=84)and non-perforation group(n=98).Multivariate Logistic regression analysis was used to analyze the risk factors of perforation in children.Results The rate of appendix perforation in 182 children was 46.15%(84/182).There were significant differences in age,onset time,preoperative vomiting,coprolith,white blood cell count,fever,rebound pain,neutrophil count,NLR ratio,lymphocyte level,CRP level,procalcitonin(PCT)level and fibrin level between perforation group and non-perforation group(P<0.05).Logistic regression analysis showed that onset time(OR=1.508,95%CI=1.028-2.213),coprolith(OR=6.720,95%CI=2.324-19.434),rebound pain(OR=8.561,95%CI=1.953-37.533),CRP level≥41.38 mg/L(OR=11.631,95%CI=3.578-37.812)and NLR ratio(OR=0.864,95%CI=0.770-0.970)were independent risk factors for perforation in children with acute appendicitis(P<0.05).Conclusion Long onset time,coprolith,rebound pain,CRP level and NLR ratio in children with acute appendicitis are independent risk factors leading to appendix perforation.Observing the above factors in early diagnosis is helpful to assess the risk of appendix perforation,thereby facilitating the determination of the treatment protocol in clinical practice.
