Observation on curative effect of hysteroscopic cold knife resection for endometrial polyps
Objective To explore the curative effect of hysteroscopic cold knife resection in the treatment of endometrial polyps.Methods A total of 122 patients with endometrial polyps admitted to our hospital were selected as the study subjects,and the patients were divided into the observation group(61 cases)and the control group(61 cases)according to the different treatment methods.Patients in the observation group were treated with hysteroscopic cold knife resection,and patients in the control group were treated with hysteroscopic electrotomy.The perioperative index,uterine spiral artery flow status[resistance index(RI),pulsatility index(PI)],menstrual status[pictorial blood loss assessment chart(PBAC)score],adverse reactions,and prognosis of patients between the two groups were compared.Results Compared with the control group,patients in the observation group had less intraoperative bleeding and shorter operation time,vaginal bleeding duration,and hospitalization time(P<0.05).Repeated measurement analysis showed that there were statistically significant differences in the RI of patients between the two groups in terms of time,inter-group and interaction(P<0.05),and statistically significant differences in PI of patients between the two groups in terms of time and interaction(P<0.05).Compared with the preoperative period,the RI and PI 1 month after surgery of patients in the two groups were lower(P<0.05),and the observation group was lower than the control group(P<0.05).Repeated measurement analysis showed that the difference in PBAC scores of patients between the two groups in terms of time was statistically significant(P<0.05);the PBAC scores of patients in the two groups 3 months after surgery were lower compared with those before sugery(P<0.05),but there was no difference in PBAC scores of patients between the two groups before surgery or 3 months after surgery(P>0.05).The total incidence of adverse reactions in the observation group was 14.75%,which was lower than that of 36.07%in the control group,and the difference was statistically significant(P<0.05).There was no statistically significant difference 6 months after surgery in the recurrence rates between the two groups(P>0.05);but compared with the control group,the observation group had higher pregnancy rates 6 months and 12 months after surgery,and a lower recurrence rate 12 months after surgery(P<0.05).Conclusion Hysteroscopic cold knife resection in treatment of endometrial polyps can effectively improve the uterine spiral artery flow,promote menstrual recovery,reduce the adverse reactions,and improve the prognosis of patients.
