Clinical features and indications of 330 poisoning patients with gastric lavage in the primary hospital
Objective To assess whether poisoning patients with gastric lavage meet the indications and to propose improvement measures in poisoning treatment by analyzing the clinical data of patients with gastrointestinal poisoning.Methods The medical records of poisoning patients in Jiangsu Province Hospital(Liyang Branch),the First Affiliated Hospital of Nanjing Medical University from January 1,2019 to December 31,2022,were retrospectively analysed.The patients with definite poison types and recorded time from ingestion to gastric lavage were screened out.According to poison toxicity and the interval time,whether the patients met the indications of gastric lavage was evaluated.Results Of the 434 patients with gastrointestinal poisoning,there were 155 males(35.7%)with a median age of 52(28,72)years.Drug poisoning and pesticide poisoning accounted for 57.4%and 36.4%respectively.76.0%cases received gastric lavage,32.3%cases used specific antidotes and 9.9%cases underwent mechanical ventilation.The proportion of gastric lavage in pesticide poisoning was higher than that in drug poisoning(91.8%vs.69.5%,P<0.001).Of the 273 patients with definite poison types and the time from ingestion to gastric lavage,16.1%cases ingested life-threatening poisoning substances,75.5%cases received gastric lavage within 2 hours.Based on toxicity and the time from ingestion to gastric lavage,36.7%cases met the indications of gastric lavage.The frequency of gastric lavage fell from 2019-2021 to 2022-2023,but there was no stastical significance(P>0.05).The coincidence rate of indications was also decreased over the period,and there was significant difference(P<0.05).Conclusions At present,gastric lavage as poisoning treatment is still widely used in primary hospitals.The majority of poisoning patients received gastric lavage within 2 hours,but the coincidence rate of indications is low.Emergency doctors should refer to toxicity,the time from ingestion to visit and contraindications for gastric lavage to determine whether the poisoning patients need gastric lavage.Emergency doctors are encouraged to receive regular training of clinical toxicology.
