With a high incidence and many complications,calcified coronary lesions constitute one of the significant challenges for interventional physicians.Presence of the lesions limits stent locating and expansion,leading to stent implantation failure,under-expansion,malapposition or other procedural complications,and simultaneously is associated with long-term target vessel failure.The development of ablation and balloon techniques has improved the preparation effect of calcified lesions than before.However,these preparation methods are still limited by calcification characteristics such as deep calcification or complete occlusion etc.In recent years,progress of the techniques has been made to relieve the aforementioned limitations.Strategy of half-way rotational atherectomy has reduced the incidence of operation-related complications.Evidence on application of excimer laser coronary atherectomy in complex calcified lesions continues to accumulate.And intracoronary lithotripsy has been used in clinical practice recently,demonstrating its advantages in deep calcified lesions.This article reviews progress on preparation techniques of calcified lesions and advances suggestions for the selection of different techniques,aiming to guide the selection of management strategies in clinical practice and improve the prognosis.
