目的 探究血清几丁质酶-3样蛋白1(YKL-40)、细胞因子信号传导抑制因子3(SOCS3)表达及其对非小细胞肺癌患者术后细菌性肺部感染的诊断价值.方法 以本院收治的87例非小细胞肺癌患者为研究对象,根据是否发生细菌性肺部感染分为感染组(n=42),非感染组(n=45).采用酶联免疫吸附法测定患者血清YKL-40、SOCS3表达水平;采用Spearman法分析YKL-40、SOCS3与患者CPIS和APACHE Ⅱ评分的相关性;采用logistic回归分析患者术后发生细菌性肺部感染的影响因素;采用受试者工作特征曲线(ROC)分析YKL-40、SOCS3对非小细胞肺癌患者术后细菌性肺部感染的诊断价值.结果 非感染组、感染组患者的CPIS评分、APACHE Ⅱ评分具有显著差异(P<0.05).与非感染组相比,感染组患者血清YKL-40呈高表达(P<0.05),SOCS3呈低表达(P<0.05).YKL-40与CPIS评分、APACHE Ⅱ评分呈正相关(P<0.05),而SOCS3与CPIS评分、APACHE Ⅱ评分呈负相关(P<0.05).血清YKL-40是患者术后细菌性肺部感染的危险因素(P<0.05),SOCS3是保护因素(P<0.05).血清YKL-40单独诊断非小细胞肺癌术后细菌性肺部感染的曲线下面积为0.881,SOCS3单独诊断的曲线下面积为0.866,二者联合诊断的曲线下面积为0.962,显著大于YKL-40(Z=1.969,P=0.049)、SOCS3(Z=2.144,P=0.032)单独诊断的曲线下面积.结论 非小细胞肺癌术后细菌性肺部感染患者血清YKL-40升高,SOCS3降低,二者联合具有较高的非小细胞肺癌术后细菌性肺部感染诊断价值.
The diagnostic value of serum YKL-40 and SOCS3 for postoperative bacterial lung infection in non-small cell lung cancer patients
Objective To investigate the expression of serum chitinase-3-like protein 1(YKL-40)and suppressor of cytokine signaling 3(SOCS3)and their diagnostic value for postoperative bacterial lung infection in non-small cell lung cancer patients.Method In this study,87 patients with non-small cell lung cancer admitted to our hospital from January 2021 to December 2023 were grouped into infection group(n=42)and non-infection group(n=45)according to whether postoperative bacterial lung infection occurred.Enzyme-linked immunosorbent assay was used to measure the expression levels of serum YKL-40 and SOCS3.Spearman method was used to analyze the correlation between YKL-40 and SOCS3 with CPIS score and APACHE Ⅱ score.Logistic regression analysis was used to analyze the influencing factors of postoperative bacterial lung infection.Receiver operating characteristic curve(ROC)was used to analyze the diagnostic value of YKL-40 and SOCS3 for postoperative bacterial lung infection in non-small cell lung cancer patients.Result There were great differences in CPIS score and APACHE Ⅱ score between the non-infection group and the infection group(P<0.05).Compared with the non-infection group,the serum YKL-40 was highly expressed(P<0.05)and SOCS3 was lowly expressed in infection group(P<0.05).YKL-40 was positively correlated with CPIS score and APACHE Ⅱ score(P<0.05),while SOCS3 was negatively correlated with CPIS score and APACHE Ⅱ score(P<0.05).Serum YKL-40 was a risk factor for postoperative bacterial lung infection(P<0.05),while SOCS3 was a protective factor(P<0.05).The area under the curve of serum YKL-40 alone in the diagnosis of bacterial lung infection after non-small cell lung cancer surgery was 0.881,and the area under the curve of SOCS3 alone was 0.866.The area under the curve of the combined diagnosis of the two was 0.962,which was greatly larger than that of YKL-40(Z=1.969,P=0.049)and SOCS3(Z=2.144,P=0.032)alone.Conclusion Serum YKL-40 is increased and SOCS3 is decreased in patients with bacterial lung infection after non-small cell lung cancer surgery.The combination of the two has a high diagnostic value for bacterial lung infection after non-small cell lung cancer surgery.
