基于教师标准化病人的妇产科OSCE考核体系的构建与实施

掌握专业临床技能是从事医疗工作的必备条件,而对医学生临床能力的客观考核及评价一直是高等医学院校教育工作者们探索的方向.文章从妇产科实习出科技能考试入手,阐述与学生学业特点、岗位需求相符合的基于教师标准化病人(teachers standardized patient,TSP)的客观结构化临床考试(objective structured clinical examination,OSCE)考核体系的构建方案及实施情况,并通过对OSCE考核体系的组织构架及临床医学专业本科实习医师OSCE出科考试成绩以及试题的难度、区分度、信度与适宜性分析,评价TSP/OSCE的有效性、可靠性和适宜性,探讨进一步优化TSP/OSCE考核体系的方案.本研究为科学、客观地评价实习医师的临床能力提供依据,也为提高医学生临床实践水平、推进"应用型"人才的培养提供参考及借鉴.
Construction and Implementation of the OSCE Assessment System Based on Teacher Standardized Patient in Obstetrics and Gynecology
The mastery of professional clinical skills is a necessary qualification for engaging in medical work,and the objective appraise and assessment of clinical ability of medical students has been the orientation of teaching staffs in medical colleges and universities to explore.This article started with the examination of obstetrics and gynecology practice,described the construction scheme and application of the evaluation system of objective structured clinical examination(OSCE)based on teacher standardized patient(TSP),which is consistent with students'academic characteristics and job needs.Moreover,the effectiveness,reliability and suitability of TSP/OSCE were evaluated by analyzing the organizational structure of the OSCE assessment system and the results of the OSCE exam for undergraduate interns in clinical medicine,as well as the difficulty,differentiation,reliability and suitability of the test questions,and the scheme to further optimize the TSP/OSCE assessment system was discussed.This study provides a basis for scientific and objective evaluation of the clinical ability of interns,and also provides reference for improving the clinical practice level of medical students and promoting the training of"application-oriented"talents.
