Risk factors of all-cause mortality in the treatment of carbapenem-resistant Gram-negative bacteria with polymyxin B
Objective This study retrospectively analyzed the clinical characteristics and risk factors of mortality in patients infected with carbapenem-resistant Gram-negative bacteria(CR-GNB)treated with polymyxin B.Methods Clinical data were collected from patients who were both suffering from CR-GNB infection in sputum or blood and receiving polymyxin B treatment for at least 48 hours during hospitalization at the first affiliated hospital of Wenzhou medical university and the Taizhou hospital of Zhejiang Province hospitals between June 1st,2017 and May 1st,2020.SPSS 25.0 software was used to compare the clinical features between the survival and death groups.Meanwhile,the risk factors for 30-day all-cause mortality in these patients were analyzed by multivariate logistic regression.Results A total of 80 patients were eligible for the study.Upon logistic regression analysis,factors associated with in-hospital mortality in these patients included the duration of polymyxin B therapy<7 days[OR=6.03,95%CI(1.28,28.41),P=0.023],the day of polymyxin B initiation[OR=1.08,95%CI(1.00,1.16),P=0.047],Charlson comorbidity index[OR=1.41,95%CI(1.09,1.81),P=0.009],and the use of vasoactive drugs[OR=3.60,95%CI(1.01,28.41),P=0.047].Conclusion Adequate duration and the early administration of polymyxin B could reduce mortality in these patients infected with CR-GNB.
