Analysis of clinical characteristics and susceptibility of infections caused by 30 strains of Streptococcus constellatus
Objective To explore the clinical features and antimicrobial susceptibility of patients infected with Streptococcus constellatus. Methods Clinical cases of infection caused by 30 strains of Streptococcus constellatus detected in the Third People's Hospital of Xinjiang Uygur Autonomous Region from November 2018 to November 2023 were reviewed, and their clinical distribution, specimen source, infection site and susceptibility to antibiotics were analyzed. Results The age of the 30 patients infected with Streptococcus constellatus ranged from 15 to 89 years old, with an average age of 50.30 years old, including 22 males(73.33%)and 8 females(26.67%).The main departments involved were otolaryngology(6 strains,20.00%),abdominal surgery(5 strains,16.67%)and hand-foot microsurgery(4 strains,13.33%).The 30 strains of Streptococcus constellatus were derived from abscess drainage(11 strains,36.67%),purulent exudate(6 strains,20.00%)and peritoneal effusion(5 strains,16.67%),respectively.The main infection sites were skin infection, chest infection and peritoneal infection, accounting for 30.00%,20.00%and 13.33%,respectively.Among the clinical features,13 cases(43.33%)had no underlying disease and 17 cases(56.67%)had an underlying disease.Among the 17 patients with underlying diseases, there were 6 cases of diabetes,4 cases of chronic obstructive pulmonary disease,4 cases of diabetes combined with hypertension, and 1 case each of cerebral infarction, chronic kidney disease and respiratory failure. In the antimicrobial susceptibility analysis of 30 strains of Streptococcus constellatus, the sensitivity rate of cephalosporins such as cefepime was 100%,and the sensitivity rate of cefotaxime and ceftriaxone was 96.67%.The penicillin sensitivity rate is 100%.The vancomycin sensitivity rate was 100%.The sensitivity rate for chloramphenicol was 93.33%.The sensitivity rate of linezolid was 100%.The sensitivity rate of tigacycline was 100%.The sensitive rates of erythromycin and clindamycin were 36.67%and 40.00%,respectively.Conclusion Streptococcus constellatus often causes skin soft tissue, thoracic and abdominal infections, and the risk of infection is higher in patients with underlying diseases.Because of its high resistance to clindamycin and erythromycin, antimicrobial drugs should be rationally selected to treat the primary lesions based on the results of drug susceptibility testing.
