摘要
恶性外周神经鞘瘤(又称为恶性周围神经鞘瘤)是一种高度恶性神经系统肿瘤,因临床罕见,临床医生对其认识尚存在不足.本文报道1例82岁男性恶性外周神经鞘瘤,因右侧肩胛部皮肤肿物进行性增大12年入院,MRI检查发现右侧肩胛部皮下占位并明显强化,术前考虑肿瘤.完善术前准备后,一期手术完整切除肿物,二期手术植皮修复创面.术后病理诊断为恶性外周神经鞘瘤.术后5个月随访,创面愈合良好,未见肿瘤复发.恶性外周神经鞘瘤侵袭性高,局部复发率高,生存率低,因此,早发现、早治疗尤为重要,扩大切除肿瘤以保证切缘阴性是主要治疗方式.
Abstract
Malignant peripheral nerve sheath tumor(also known as malignant peripheral nerve sheath tumor)is a highly malignant tumor of the nervous system.Because of its clinical rarity,clinicians still have insufficient understanding of it.This paper reports a case of malignant peripheral nerve sheath tumor in an 82-year-old man.He was admitted to hospital for 12 years due to progressive enlargement of a mass in the right scapula.MRI found a space occupying in the right scapula with significant enhancement,considering a tumor preoperatively.After completing preoperative preparation,the mass was completely removed in the first stage surgery,and the wound was repaired in the second stage surgery.The postoperative pathological diagnosis was malignant peripheral nerve sheath tumor.Five months after surgery,the wound healed well,and no tumor recurrence was found.Malignant peripheral nerve sheath tumor is highly invasive,with a high local recurrence rate and a low survival rate.Therefore,early detection and early treatment are particularly important.Extended resection of the tumor to ensure negative margins is the main treatment method.
