Application of microwire combined with microcatheter assisted technology in the embolization of ruptured intracranial aneurysms associated with important branch vessels originating from the aneurysm neck
Objective To explore the application value of the microwire combined with microcatheter-assisted technology for branch vessel protection in endovascular treatment of ruptured intracranial aneurysms associated with important branch vessels originating from the aneurysm neck.Methods The clinical data of 8 patients with ruptured intracranial aneurysms associated with important branch vessels originating from the aneurysm neck treated by endovascular embolization from June 2021 to June 2022 were retrospectively analyzed.During the operation,a microwire carrying a microcatheter was placed in the branch vessel at the aneurysm neck,and the tension was utilized to protect the branch vessel and assist in coil embolization.Results All cases successfully completed the operation,and no complications such as aneurysm rupture or vascular embolization occurred during the operation.The immediate postoperative angiography showed that Raymond grade 1 was achieved in 6 cases and grade 2 in 2,and the blood flow of all branch vessels was patent after the operation.The 3-6-month postoperative follow-up showed that all aneurysms were occluded on DSA without recurrence,and the branch vessels were patent;a GOS score of 4~5 was achieved in 7 cases and a GOS score of 3 in 1.Conclusions For patients with ruptured intracranial aneurysms associated with important branch vessels originating from the aneurysm neck,the use of the microwire combined with the microcatheter-assisted technique for branch vessel protection in coil embolization is feasible,safe,and has a good effectiveness.
