Expert consensus on clinical parenteral nutrition prescription review

摘要
肠外营养(parenteral nutrition,PN)处方因药物组分多,存在药物间配伍与营养素配比合理性问题,进而影响营养混合液稳定性、相容性及临床应用的安全性、有效性.若处方不当很可能对患者造成严重伤害,因此,处方审核是确保用药安全的前提.为进一步规范PN处方的合理性,加强处方审核工作,天津市医学会肠外肠内营养学分会组织国内近百位相关领域的专家,基于现有的循证医学和药学证据,围绕适应证审核、药物选择的合理性、药物间配伍问题、营养液的稳定性等PN处方审核要点进行阐述,提出11个问题,形成了 24条推荐意见,最终形成该项专家共识,旨在为临床PN治疗安全、有效、经济、适宜的规范应用提供参考.
Abstract
Parenteral nutrition(PN)prescriptions usually consist of a large number of drugs,resulting in the concerns of compatibility between drugs and rationality of nutrient ratio,which will affect the stability,compatibility,safety and effectiveness of clinical application of nutrient mixture.Improper prescription is likely to cause serious harm to patients,rendering prescription review necessary to ensure drug safety.In order to further standardize the rationality of PN prescriptions and strengthen prescription review,Tianjin Medical Association Branch of Parenteral and Enteral Nutrition held a discussion with a panel of nearly 100 domestic experts in related fields to elaborate the keys of PN prescription review.The discussion was based on existing evidence in evidence-based medicine and pharmaceutical science,and covered indications review,drug selection,cross-component compatibility,and stability of nutrient solution.Eventually,11 questions and 24 recommendations were put forward to form the Expert Consensus on Clinical Parenteral Nutrition Prescription Review,aiming to inform and facilitate safe,effective,economical and appropriate standard application of clinical PN treatment.
