摘要
目的:分析 2020-2023 年广州市男性婚检人群中乙型肝炎病毒(HBV)和梅毒的感染率及影响因素.方法:选取 2020 年 1 月至 2023 年 12 月在我院进行婚检的 22 596 名男性作为研究对象.通过问卷调查收集其基本信息,包括年龄、文化程度、性行为史等,并检测其血清中的乙肝表面抗原(HBsAg)和梅毒螺旋体抗体.采用多因素Logistic回归分析法,探讨影响HBV和梅毒感染的相关因素.结果:广州市男性婚检人群的HBV感染率为 1.50%(340/22 596),梅毒感染率为 2.25%(508/22 596).多因素Logistic回归分析显示,有乙肝疫苗接种史是HBV感染的保护因素(P<0.05);而文化程度较低(高中/中专及以下)、乙肝家族史、丙型肝炎史是HBV感染的危险因素(P<0.05).文化程度较低(高中/中专及以下)、与同性发生肛交性行为、性伴侣数量≥2 人、有吸毒行为、最近 1 年患过性病、HIV抗体阳性、未坚持使用安全套是梅毒感染的危险因素(P<0.05).结论:广州市2020-2023 年男性婚检人群HBV、梅毒感染率较高,且受多种因素的影响.
Abstract
Objective:To analyze the infection rates and influencing factors of hepatitis B virus(HBV)and syphilis among male premarital check-ups in Guangzhou from 2020 to 2023.Methods:A total of 22 596 men who underwent premarital medical examinations in our hospital from January 2020 to December 2023 were selected as the research subjects.Their basic information,including age,educational level,and sexual behav-ior history were collected through questionnaire surveys,and hepatitis B surface antigen(HBsAg)and Trepo-nema pallidum antibodies in their serum were detected.Multivariate Logistic regression analysis was used to analyze the related factors.Results:HBV infection rate was 1.50%(340/22 596)and syphilis infection rate was 2.25%(508/22 596)among male premarital check-up population in Guangzhou.Multiple Logistic regres-sion analysis showed that hepatitis B vaccination history was a protective factor for HBV infection(P<0.05).However,lower educational attainment(high school/technical secondary school and below),family history of hepatitis B or hepatitis C were risk factors for HBV infection(P<0.05).Lower educational attainment(high school/technical secondary school and below),having anal sex with the same sex,having 2 or more sexual partners,admitting drug use,having a sexually transmitted diseases in the past 1 year,positive HIV antibody,failure to consistently use condoms were the risk factors for syphilis infection(P<0.05).Conclusion:The in-fection rate of HBV and syphilis among male premarital check-ups in Guangzhou from 2020 to 2023 is high,and HBV and syphilis infection is affected by many factors.
