Burden of noncommunicable diseases among children and adolescents in China and its provinces from 1990 to 2021
Objective To analyze the disease burden and changes of major chronic diseases in children and adolescents in China and its provinces from 1990 to 2021,and provide the scientific basis for optimizing the prevention and treatment of chronic diseases in children and adolescents.Methods The data were from the results of China and provinces study in Global Burden of Disease 2021,the indicators of mortality,years of life lost(YLL),years lived with disability(YLD),and disability-adjusted life years(DALY)were used to analyze the current status and changes of major chronic disease burden of children and adolescents in China from 1990 to 2021.The data were analyzed using software SAS 9.4.Joinpoint 5.2.0 software was used to calculate the average annual percentage change(AAPC)through a log-linear regression model,the long-term trends in disease burden from 1990 to 2021 were analyzed.Results In 2021,the mortality rate and DALY rate of chronic diseases among children and adolescents in China were 15.1/105 and 3 952.9/105,respectively.The mortality rate and DALY rate in males were 17.2/1 05 and 4 015.8/105,respectively;the mortality rate and DALY rate in females were 12.8/105 and 3 880.8/1 05,respectively.Analysis by age group showed that the mortality(34.7/105)of children<5 years old was the highest.Compared with 1990,the mortality rate of chronic diseases among children and adolescents in China decreased by 79.6%in 2021.The top 5 causes of death were tumor,cardiovascular diseases,neurological diseases,digestive system diseases,diabetes and kidney diseases.The DALY rate decreased by 57.2%.The DALY ranking of mental disorders,skin and subcutaneous diseases,neurological disorders,and sensory organ disorders among males and females all rose,the first diseases leading to DALY was mental disorders.There was the difference of death level among children and adolescents in different regions,the age-standardized mortality in Xizang,Qinghai and Xinjiang were higher.Conclusion The burden of chronic diseases among children and adolescents in China from 1990 to 2021 showed decrease trend,but the prevention and control situation remains grim.Targeted prevention strategies should be adopted,the attention to the mental health status of children and adolescents should be strengthened,and the preventive and health care work for children<5 years old should be strengthened.
