Changes in the disease burden of injury death among elderly(≥60 years old)in Chongqing from 2012 to 2023
Objective To analyze the change trend in the disease burden of injury death among elderly(≥60 years old)in Chongqing from 2012 to 2023,and provide the suggestion for conducting the injury prevention among the elderly.Methods Cases of death simply because of injury served as death case according to cause of death surveillance data from 2012 to 2023.The mortality,standardized mortality rate(SMR),years of life lost(YLL),average years of life lost(AYLL)and the composition ratio of different causes of death were calculated.The x2 test was used to compare the mortality,the annual percentage change(APC)and average annual percentage change(AAPC)were used to indicate the change trend of rates.The used software was SPSS 26.0.Results The mortality and SMR of injury among elderly(≥60 years old)in Chongqing in 2012 were 106.55/105 and 108.28/105,respectively;the mortality and SMR of injury among elderly(≥ 60 years old)in Chongqing in 2023 were 104.76/105 and 93.14/105,respectively.AAPC values were-0.70%and-1.49%,respectively(P>0.05).From 2012 to 2016,the injury mortality rate increased by 5.18%annually;from 2016 to 2023,the injury mortality rate decreased by 3.25%annually;there were significant difference for the injury mortality rate(P<0.05).From 2012 to 2017,the SMR increased by 4.09%annually;from 2017 to 2023,the SMR decreased by 5.64%annually;there were significant difference for SMR(P<0.05).The mortality rate of male elderly was significantly higher than that of female elderly over the years(P<0.05).Apart from 2015,2018 and 2019,the injury mortality rates on elderly in rural area were significantly higher that those in urban area from 2012 to 2023(P<0.05).Injury mortality in elderly increased with age,but in 60-64,65-69 and 70-74 age group,the injury mortality rate decreased by 5.92%,2.57%and 2.27%annually,respectively;there were significant difference for death trend(P<0.05).The YLL rate of the elderly due to injury in Chongqing decreased from 19.05‰ in 2012 to 16.28‰ in 2023,and the AAPC was-1.73%,with a statistically significant trend(P<0.05).The AYLL rate of the elderly due to injury in Chongqing decreased from 17.88 years in 2012 to 15.54 years in 2023,and the AAPC was-1.37%,with a statistically significant trend(P<0.05).The first three injury deaths among the elderly(≥60 years old)in Chongqing were falls,traffic accidents and suicide.The proportion of deaths due to falls increased from 28.85%in 2012 to 50.55%in 2023,the proportion of deaths due to traffic accidents decreased from 23.53%in 2012 to 15.42%in 2023,and the proportion of deaths due to suicide decreased from 15.34%in 2012 to 12.34%in 2023,with statistical trends(AAPC values were 5.98%,-4.67%and-2.85%,respectively,P<0.01).Conclusion Injury mortality among elderly(≥60 years old)in Chongqing is high.Male,rural areas and the elderly are the focus of injury prevention and control.
