十二指肠神经内分泌瘤伴肝脏与淋巴结转移的转录组学差异分析

目的 探索引起十二指肠神经内分泌瘤合并肝脏和淋巴结转移的关键基因及转移的潜在分子机制.方法 对1例罕见的合并肝脏和淋巴结转移的十二指肠神经内分泌瘤患者的癌旁十二指肠上皮组织(简称"正常组织")、原发病灶、肝转移灶和淋巴结转移灶进行测序分析,筛选不同病灶之间的差异表达基因(differentially expressed genes,DEGs)并进行功能富集分析.结果 以正常组织作为对照,筛选出了只在肝转移灶中表达的DEGs有2 053个,只在淋巴结转移灶中表达的DEGs有742个,分别只在这两种转移灶组织中表达且差异表达倍数前5位的基因分别是ORM1、C4BPA、AHSG、C9和LBP以及ABHD12B、AC100850.1、HOXC9、AC083967.1和HOXC8.京都基因与基因组百科全书富集分析发现,DEGs主要富集于磷脂酰肌醇3激酶/蛋白激酶B通路、丝裂原活化蛋白激酶通路、人类乳头状瘤病毒感染等.结论 从本研究中这例合并肝脏和淋巴结转移的十二指肠神经内分泌瘤患者中发现了多个存在于转移病灶中的DEGs和通路,这为治疗和预防十二指肠神经内分泌瘤提供了新的方向.
Analysis of transcriptomic differences of duodenal neuroendocrine tumor accompanied by liver and lymph node metastases
Objective To explore the key genes and potential molecular mechanisms of liver and lymph node metastases relevant to duodenal neuroendocrine tumors(DNET).Methods The tissues of paracancerous duodenal epithelial,primary lesion,liver metastasis lesion,and lymph node metastasis lesion of a rare DNET accompanied by liver and lymph node metastases were sequenced and analyzed.The differentially expressed genes(DEGs)were screened for different tissues and the functional enrichment analysis was performed.Results The tissues of paracancerous duodenal epithelial was used as the control,a total of 2 053 DEGs expressed only in the liver metastases lesion tissues and 742 DEGs expressed only in the lymph node metastases lesion tissues were screened out,and the top 5 genes expressed in the liver metastases lesion tissues were ORM1,C4BPA,AHSG,C9,and LBP,which in the lymph node metastases lesion tissues were ABHD12B,AC100850.1,HOXC9,AC083967.1,and HOXC8.Kyoto Encyclopedia of Genes and Genomes enrichment analysis found that the DEGs were mainly enriched in the phosphatidylinosiol 3 kinase/protein kinase B pathway,mitogen-activated protein kinase pathway,human papillomavirus infection,etc.Conclusion Multiple DEGs and pathways in metastatic lesions are found in this patient with DNET accompanied by liver metastasis and lymph node metastasis,which provides a new direction for treatment and prophylaxis of DNET.
