Analysis on the application value of the framework of the Omaha system case management model in diabetes pharmaceutical care
Objective To evaluate the Omaha system analysis framework for case management model of the value of pharmaceutical care in diabetes. Methods A total of 304 patients with diabetes mellitus were selected from December 2015 to December 2016. The patients were divided into control group and intervention group according to the randomized grouping method. The control group was given routine treatment, and the intervention group gave the Omaha system as a case management model. The patients were followed up for 6 months after discharge, and the blood glucose control and medication rationality, compliance and adverse reactions were observed. Results During the study period, four patients withdrew from the study plan due to illness or non-compliance. The compliance rate was 96.67% in the intervention group and higher than 82.67%in the control group (P<0.05). The rate of blood glucose control and disease-related knowledge were better than those in the control group, and the difference was statistically significant (P<0.05). Conclusion The Omaha system case management model for the framework is a kind of diabetes pharmaceutical care mode suitable for China's national conditions, to improve the therapeutic compliance of patients, improve the rationality of the patient's medication, promote the patient blood sugar control, used to be promoted.
