Association of enlarged perivascular space and neutrophil/lymphocyte rate with disease severity in patients with neuromyelitis optica spectrum disorders
Objective To investigate the relationship between enlarged perivascular space(EPVS),neutrophil/lymphocyte rate(NLR)and disease severity in patients with neuromyelitis spectrum disorders(NMOSD),and their diagnostic value in severe neurological deficits in patients with NMOSD.Methods A total of 164 patients with NMOSD admitted to the First Affiliated Hospital of Zhengzhou University from November 2019 to April 2023 were retrospectively selected.The patients were divided into mild[the expanded disability status scale(EDSS)<5,independent walking was not limited]and severe groups(EDSS ≥ 5,independent walking was limited)according to EDSS score.The differences in clinical data between the two groups were compared.Spearman rank correlation analysis was used to explore the correlation between EPVS,NLR and EDSS score in NMOSD patients.Multivariate logistics regression analysis was used to investigate the influencing factors of severe neurological deficits in NMOSD patients,and receiver operating curve(ROC)was used to evaluate the diagnostic value of EPVS and NLR in identifying severe neurological deficits in patients with NMOSD.Results The total EPVS number and NLR level in severe group were higher than those in mild group(P<0.05).Spearman rank correlation analysis showed that the total number of EPVS(rs=0.374,P<0.01)and NLR(rs=0.329,P<0.01)were positively correlated with EDSS score in NMOSD patients.Multivariate logistic regression analysis showed that age(OR=1.082,95%CI:1.046-1.120;P<0.01),total number of EPVS(OR=1.083,95%CI:1.008-1.663;P<0.05),NLR(OR=1.334,95%CI:1.077-1.653;P<0.01),spinal lesion length(OR=1.168,95%CI:1.073-1.272;P<0.01)were the influencing factors of severe neurological impairment in NMOSD patients.The area under the ROC curve(AUC)of total EPVS combined with NLR was 0.775(95%CI:0.696-0.854,P<0.01),the sensitivity and specificity were 80.00%and 71.43%respectively.Conclusions EPVS and NLR are positively correlated with disease severity in NMOSD patients.They can be used to identify severe neurological impairment to some extent,and the combined diagnosis efficiency is higher.
