Relationship Between Folic Acid Nutritional Status and The Occurrence of Depression in Diabetic Patients in Late Pregnancy
[Objective]To investigate the relationship between folic acid nutritional status and the occurrence of depression in diabetic patients in late pregnancy.[Method]Totally 156 cases of diabetes mellitus patients in late pregnancy who underwent obstetric examination in the Second People's Hospital of Hefei City from January 2023 to January 2024 were collected as study subjects.The general conditions of pregnant women in late pregnancy were collected through obstetric examination information and questionnaires,and early morning fasting venous blood was collected from patients with gestational diabetes mellitus in late pregnancy and sent to our department for fasting blood glucose and fasting insulin level testing.Folic acid nutritional status was measured by microparticle chemiluminescence for erythrocyte folic acid levels.Patients'depression was assessed based on the Self-Depression Scale(SDS),and patients were divided into the normal group(no depression,SDS<53 points)and the observation group(depression,SDS ≥ 53 points)based on their SDS scores and scoring criteria.[Result]There was no difference in age,pre-pregnancy BMI,place of residence,cultural level,workload during pregnancy,planned mode of delivery,whether or not the pregnancy was singleton,number of previous births,and fasting insulin between the patients in the observation group and the normal group(P>0.05).In contrast,the proportion of patients in the observation group with no physical activity during pregnancy,no planned pregnancy,and adverse pregnancy history was higher than that of the normal group,and the fasting blood glucose level was significantly higher than that of the normal group[(7.58±1.37)vs.(6.51±1.42)mmol/L,P<0.05].The erythrocyte folic acid value of the observation group was(228.43±113.06)ng/mL,which was significantly lower than that of the normal group[(312.65±123.38)ng/mL,P<0.001],and the incidence of folate deficiency was higher in the observation group than in the normal group(61.29%vs.25.60%,P<0.001).Multifactorial analysis found that fasting blood glucose,no physical activity during pregnancy,unplanned pregnancy,history of adverse pregnancy and presence of folate deficiency were independent risk factors for the development of depression in patients with diabetes mellitus in late pregnancy(P<0.05).[Conclusion]The incidence of depression is higher in patients with diabetes mellitus in late pregnancy,and folic acid deficiency,increased fasting glucose,lack of physical activity dur-ing pregnancy,unplanned pregnancy,and a history of adverse pregnancy are independent risk factors for the development of depression in patients with diabetes mellitus in late pregnancy.The nutritional status of folic acid in diabetic patients in late pregnancy should be specifi-cally addressed to reduce the development of depression.
