目的 通过数字化方法探讨上中切牙的牙冠形态、牙龈厚度、空间因素(包括牙冠长度、牙冠宽长比、龈缘交角)与上中切牙间龈乳头高度的联系,为前牙种植修复设计提供理论参考。方法 选取于2022年12月至2023年6月在郑州大学第一附属医院口腔医学中心就诊的120例前牙区组织健康而后牙区需做修复治疗的患者作为研究对象,收集研究对象的口内扫描信息和锥形束CT资料,利用3Shape TRIOS3进行口内扫描和锥形束CT数据的拟合配准,测量上中切牙龈缘下牙龈厚度,并利用Geomagic Control X 64软件,区分上中切牙牙冠形态,测量牙冠长度和宽度、龈缘交角、龈乳头高度和宽度。按照牙冠形态将研究对象分为3组(尖圆形组、卵圆形组、方圆形组),对比3组的测量指标并分析研究对象的各测量指标之间的相关性。结果 不同形态牙冠间的牙龈厚度、龈乳头高度差异均有统计学意义(P<0。05):方圆形组的牙龈厚度(龈缘下4 mm处)大于尖圆形组、卵圆形组,而龈乳头高度小于尖圆形组、卵圆形组;尖圆形组的牙龈厚度(龈缘下2 mm处)小于卵圆形组,而龈乳头高度则大于卵圆形组、方圆形组。方圆形组的龈缘交角、牙冠宽长比大于尖圆形组、卵圆形组,而牙冠长度小于尖圆形组、卵圆形组,差异均有统计学意义(P<0。05)。牙冠长度与龈乳头高度呈有统计学意义的正相关性,牙龈厚度、牙冠宽长比和龈缘交角与龈乳头高度呈有统计学意义的负相关性(P<0。05)。结论 天然状态下,牙龈厚度与龈乳头高度之间的负相关性可能归因于牙龈厚度与牙冠形态、空间因素的联系,不同形态牙冠之间空间因素的差异可能是不同形态牙冠的龈乳头高度之间存在差异的重要原因。
Digital measurement and analysis of influencing factors of gingival papilla between upper central incisors
Objective To explore the relationship between the crown morphology,gingival thickness,and space factors(including crown length,crown width-to-length ratio,and gingival margin angle)of the maxillary central incisors and the height of the interproximal papillae,in order to provide theoretical guidance for the design of anterior dental implants.Methods Totally 120 patients who received treatment at the Oral Medicine Center of the First Affiliated Hospital of Zhengzhou University from December 2022 to June 2023 were selected.These patients had healthy anterior teeth but re-quired restorative treatment for their posterior teeth.The patients'intraoral scanning data and cone beam CT images were collected,and 3Shape TRIOS3 was used for data registration.We measured the gingival thickness below the gingival mar-gin of the maxillary central incisors and utilized Geomagic Control X 64 software to classify the crown morphology of the incisors,measuring crown length,width,the gingival margin angle and the height and width of the papilla.The subjects were categorized into three groups based on crown shape(triangular,ovoid,and square),and we compared the measure-ment parameters among these groups,subsequently analyzing the correlations among the various measurements.Results The differences in gingival thickness and papilla height among various crown shapes were statistically significant(P<0.05).The square group exhibited greater gingival thick-ness than the triangular and ovoid groups at a depth of 4 mm below the gum line,but lower papilla height com-pared to the triangular and ovoid groups.The triangular group had less gingival thickness than the ovoid group at a depth of 2 mm below the gum line,while its papilla height was greater than that of both the ovoid and square groups.Additional-ly,the square group's gingival margin angle and crown width-to-length ratio were larger than those of the triangular and ovoid groups,while its crown length was shorter than that of the triangular and ovoid groups,with all differences being statistically significant(P<0.05).There was a statistically significant positive correlation between crown length and pa-pilla height,and a statistically significant negative correlation between gingival thickness,crown width-to-length ratio,gingival margin angle and papilla height(P<0.05).Conclusion In their natural state,the negative correlation between gingival thickness and papilla height may be attributed to the relationship between gingival thickness and crown morphol-ogy as well as space factors.Variations in space factors associated with different crown morphologies could significantly contribute to the differences in papilla height among these morphologies.
