Endoscopic ultrasound-guided coil placement combined with tissue glue injection in the treatment of gastric variceal bleeding with gastrorenal shunt/splenorenal shunt
Objective To explore the effectiveness and safety of EUS-guided coil placement combined with glue injection in the treatment of gastric varicose bleeding combined with GRS/SRS.Methods The hemostasis success rate,rebleeding rate,and ectopic embolization rate of the above patients in Beijing Friendship Hospital,Capital Medical University from December 2017 to May 2023 were retrospectively analyzed.Results A total of 48 patients were included in the study,with a male-female ratio of 20:28,and an average age of 62.7(39.0-71.0)years.Among them,15 patients had GRS,29 patients had SRS,and 4 patients had GRS combined with SRS.The average maximum diameter of the shunt channel was 1.5 cm,and the average minimum diameter was 0.7 cm.The operation success rate was 100%,and the hemostasis success rate was 97.9%.The average diameter of the coil was 11.3 mm,and the average number of coils inserted was 1.3.The average injection volume of tissue glue was 6.3 mL.The average follow-up was 2.9 years,the rebleeding rate was 18.8%,and the incidence of ectopic embolism was 4.2%.Conclusion EUS-guided coil combined with tissue glue has a good effect on the treatment of gastric varicose bleeding combined with GRS/SRS,and the incidence of complications is low.
