摘要

目的 探讨绝经后2型糖尿病女性甲状腺参数与DKD的关系.方法 对安徽医科大学第二附属医院内分泌科2016年1月至2021年12月收治的365例绝经后2型糖尿病女性患者进行回顾性分析.收集其个人史、生活方式、身体测量指标、血尿生化等指标,计算促甲状腺激素指数(TSHI)、促甲状腺激素细胞-T4抵抗指数(TT4RI)、外周脱碘酶的总和(SPINA-GD)、甲状腺分泌能力(SPINA-GT).logistic回归模型分析绝经后女性甲状腺相关参数与DKD关系.结果 游离三碘甲腺原氨酸(FT3)与估算肾小球滤过率(eGFR)成正相关,与血肌酐(Cr)成负相关,P<0.05.SPINA-GD、SPINA-GT分别与eGFR成正相关,与尿白蛋白/肌酐比率(UACR)、Cr成负相关,P<0.05.TSHI和TT4RI分别与UACR、Cr成正相关,与eGFR成负相关,P<0.05.促甲状腺激素(TSH)与UACR成正相关,与eGFR成负相关,P<0.05.logistic回归分析表明,校正混杂因素后SPINA-GD(OR 0.87,95%CI:0.80~0.93)为DKD保护因素.结论 在绝经后2型糖尿病女性中,SPINA-GD水平升高降低糖尿病肾脏疾病风险.

Abstract

Objective To analyze the association of thyroid parameters with kidney disorders in post-menopausal women with type 2 from diabetes.Methods We conducted a retrospective study among 365 post-menopausal women with type 2 diabetes from January 2016 to December 2021 in the second affiliated hospital of Anhui medical unirersity and collected the data of personal history,personal lifestyles,physical measurment and laboratory indicators.SPINA-GD,SPINA-GT,TSHI,TT4RI,were calculated.The association between thyroid parameters and DKD was analyzed using logistic regression models.Results Free triiodothyronine(FT3)was positively correlated with eGFR and negatively correlated with creatinine,P<0.05.SPINA-GD and SPINA-GT were positively correlated with eGFR and negatively correlated with UACR and Cr,P<0.05.TSHI and TT4RI were positively correlated with UACR and Cr and negatively correlated with eGFR,P<0.05.Thyroid-stimulating hormone(TSH)was positively correlated with UACR and negatively correlated with eGFR,P<0.05.After adjusting for confounding factors,logistic regression analysis showed that SPINA-GD(OR 0.88,95%CI:0.82-0.96)was a protective factor of DKD.Conclusion Higher SPINA-GD can reduce the risk of DKD in postmenopausal womens with type 2 diabetes.
