Implementation strategies of lung protective ventilation in patients with non-invasive ventilation
With the exploration of the lung injury mechanism during mechanical ventilation,lung protective ventilation strategies have been widely used in patients receiving invasive ventilation,and the importance of lung protective ventilation during non-invasive ventilation has also been increasingly recognized.The implementation strategies of lung protective ventilation in patients with non-invasive ventilation includse protection of restricting respiratory mechanics achieved by monitoring and controlling respiratory drive/effort,protection of improving patient-ventilator synchronization by optimizing the interface and parameter or mode of the ventilator,and protection of reducing the heterogeneity by collaborating with awake prone position.The monitoring and control of respiratory effort or drive is the core of the implementation of lung protective ventilation during non-invasive ventilation.The mechanism of lung injury during non-invasive ventilation and the effectiveness of the lung protective interventions need further studies.The concept of lung protective ventilation for patients with non-invasive ventilation is not only a supplement to the management strategy in new definition of ARDS,but may also promote further studies on the mechanisms and strategies mentioned above.
