摘要
急性胰腺炎(AP)病程复杂多变,并发症多,治疗难度高,预后差别大.因此,规范、全面、准确的影像学报告不仅对其诊断、分类和严重程度评估起着重要作用,还利于不同机构间检查互认,增强数据的科学价值.然而,目前大部分AP影像学报告仍为自由语言报告,价值有限.增强CT和MRI可综合评估AP病人胰腺及胰周坏死程度及积液情况,选择合适的影像检查手段有利于准确判断AP的严重程度.AP影像学结构化报告应包括胰腺体积、实质坏死、胰外坏死、局部并发症、其他并发症、合并症和影像学诊断,并根据改良的CT严重指数进行影像学严重程度评估.
Abstract
Acute pancreatitis(AP)is characterized by a complex course with numerous complications,challenging treatment,and varied prognosis.Therefore,standardized,comprehensive,and accurate imaging reports not only play an important role in diagnosis,classification,and severity assessment but also facilitate the mutual recognition of examinations among different institutions and enhance the scientific value of data.However,at present,most AP imaging reports are still free language reports with limited value.Enhanced CT and/or MRI can comprehensively evaluate the extent of pancreatic and peripancreatic necrosis and fluid collection in patients with AP.Choosing the appropriate imaging modality is beneficial for evaluating the severity of AP.Structured imaging reports for AP should include pancreatic volume,parenchymal necrosis,extrapancreatic necrosis,local complications,other complications,comorbidities,and imaging diagnosis.The severity of acute pancreatitis can be assessed based on the modified CT severity index.
