Observation on the effect of sertraline combined with eszopiclone in the treatment of elderly depression with insomnia
Objective To observe the clinical effect of sertraline combined with eszopiclone in the treatment of elderly depression with insomnia.Methods Selected 76 elderly patients with depression and insomnia were divided into an observation group(40 cases)and a control group(36 cases)according to random number table method.The control group was treated with sertraline,and the observation group was treated with sertraline and eszopiclone.The daily effective dose and onset time of the two groups were recorded,and the clinical efficacy,the occurrence of adverse reactions,the score of Hamilton Depression scale(HAMD)and sleep effect before and after treatment were compared between the two groups.Results The observation group had 2 drop-out cases 1 week later,and the control group had 1 drop-out case 1 week later,and the rest of the patients completed the 6-week course.The data of the remaining 73 patients were completely analyzed.In the observation group,the average daily effective dose of sertraline and eszopiclone were(61.58±18.08)and(1.52±0.72)mg,respectively,and the average onset time was 1.5 weeks.In the control group,the average daily effective dose of sertraline was(64.64±19.52)mg,and the average onset time was 2 weeks.There was no statistically significant difference in the average daily effective dose of sertraline between the two groups(P>0.05).In the control group,11 cases were cured,11 cases were significantly effective,4 cases were effective,9 cases were ineffective,and the total effective rate was 74.3%.In the observation group,20 cases were cured,14 cases were significantly effective,2 cases were effective,2 cases were ineffective,and the total effective rate was 94.7%.The total effective rate of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).After 1,2,4 and 6 weeks of treatment,HAMD scores in both groups were lower than those before treatment in this group;after 4 and 6 weeks of treatment,HAMD scores in the observation group were(14.00±3.87)and(9.71±3.18)points,respectively,which were significantly lower than those of(16.40±2.87)and(13.11±2.47)points in the control group;the difference was statistically significant(P<0.05).After 6 weeks of treatment,the Pittsburgh Sleep Quality Index(PSQI)score of(8.28±2.56)points in the observation group was lower than that of(11.35±2.17)points in the control group,and the difference was statistically significant(P<0.05).During treatment,dizziness,lethargy,anorexia,nausea,dry mouth,diarrhea,insomnia and tachycardia occurred,and the incidence of adverse reactions was 21.1%(8/38)in the observation group and 17.1%(6/35)in the treatment group,with no statistical significance(P>0.05).After symptomatic treatment,they were all quickly relieved,and blood routine,liver function,kidney function and other laboratory tests were normal.Conclusion In the treatment of elderly depression with insomnia,sertraline combined with eszopiclone is easy to take,and has good tolerance and no drug dependence significant effect and high safety,so it is worthy of clinical promotion.
