营养干预对非小细胞肺癌患者化疗期间营养水平的影响观察

目的 探讨营养干预对非小细胞肺癌(NSCLC)患者化疗期间营养水平的影响.方法 98例非小细胞肺癌患者作为研究对象,随机分为研究组和对照组,每组 49 例.对照组给予常规干预,研究组给予运动干预及营养干预.对比两组患者的生化指标、体重及体质量指数.结果 化疗前和化疗第1~4 周期,对照组的白细胞计数分别为(6.41±2.69)×109/L、(6.28±3.44)×109/L、(5.23±1.20)×109/L、(5.21±1.27)×109/L、(4.06±2.36)×109/L,中性粒细胞绝对值分别为(4.39±2.30)×109/L、(4.30±2.05)×109/L、(4.10±0.33)×109/L、(3.77±0.46)×109/L、(3.36±1.28)×109/L,总淋巴细胞绝对值分别为(1.42±0.66)×109/L、(1.34±0.72)×109/L、(1.00±0.68)×109/L、(1.00±0.46)×109/L、(0.98±0.33)×109/L,血小板计数分别为(221.22±90.93)×109/L、(215.98±90.23)×109/L、(202.39±21.12)×109/L、(192.60±32.76)×109/L、(187.44±15.69)×109/L,血红蛋白分别为(121.20±25.16)、(120.34±16.68)、(112.23±15.12)、(108.30±16.20)、(103.98±14.25)g/L,总蛋白分别为(65.33±4.35)、(65.58±5.66)、(60.65±6.75)、(58.89±5.69)、(57.34±6.23)g/L,白蛋白分别为(44.36±5.24)、(43.88±5.79)、(39.03±6.75)、(37.85±5.69)、(26.31±6.28)g/L;研究组的白细胞计数分别为(6.20±2.36)×109/L、(6.88±5.64)×109/L、(5.95±1.45)×109/L、(5.79±1.56)×109/L、(5.63±4.36)×109/L,中性粒细胞绝对值分别为(4.35±2.56)×109/L、(4.86±5.36)×109/L、(4.30±0.56)×109/L、(4.12±0.34)×109/L、(3.90±1.30)×109/L,总淋巴细胞绝对值分别为(1.44±0.86)×109/L、(1.46±0.77)×109/L、(1.33±0.68)×109/L、(1.26±0.77)×109/L、(1.14±0.42)×109/L,血小板计数分别为(224.12±94.23)×109/L、(222.20±82.59)×109/L、(220.19±33.31)×109/L、(205.43±25.33)×109/L、(197.20±24.26)×109/L,血红蛋白分别为(123.55±23.95)、(122.26±21.98)、(119.24±15.23)、(116.20±22.13)、(114.01±20.46)g/L,总蛋白分别为(65.12±4.76)、(65.68±5.26)、(63.45±5.06)、(61.30±4.03)、(60.11±4.82)g/L,白蛋白分别为(43.25±6.69)、(44.98±6.27)、(41.65±6.03)、(40.20±5.03)、(39.25±6.03)g/L.两组患者在化疗前及化疗第 1 周期的白细胞计数、中性粒细胞绝对值、总淋巴细胞绝对值、血小板计数、血红蛋白、总蛋白、白蛋白对比差异均无统计学意义(P>0.05);化疗第 2~4 周期,研究组的白细胞计数、中性粒细胞绝对值、总淋巴细胞绝对值、血小板计数、血红蛋白、总蛋白、白蛋白均优于对照组,差异有统计学意义(P<0.05).化疗前和化疗第 1~4 周期,对照组的体重分别为(65.02±11.34)、(65.29±11.26)、(61.00±7.26)、(59.03±9.36)、(58.69±8.46)kg,体质量指数分别为(23.56±2.99)、(23.87±2.11)、(22.26±2.29)、(21.16±2.59)、(21.06±2.21)kg/m2;研究组体重分别为(65.30±10.58)、(65.29±10.75)、(64.55±10.06)、(63.88±10.74)、(62.56±9.20)kg,体质量指数分别为(23.56±2.96)、(23.54±2.75)、(23.41±2.48)、(23.01±2.45)、(22.12±2.05)kg/m2.两组患者在化疗前及化疗第 1 周期的体重、体质量指数对比差异均无统计学意义(P>0.05);化疗第 2~4 周期,研究组的体重、体质量指数均优于对照组,差异有统计学意义(P<0.05).结论 营养干预对非小细胞肺癌患者化疗期间营养水平具有一定临床价值,值得广泛推广以及应用.
Effect observation of nutritional intervention on nutritional level of non-small cell lung cancer patients during chemotherapy
Objective To discuss the effect of nutritional intervention on nutritional level of non-small cell lung cancer(NSCLC)patients during chemotherapy.Methods 98 patients with non-small cell lung cancer were selected as the study subjects.They were randomly divided into a study group and a control group,with 49 cases in each group.The control group was given conventional intervention,while the study group was given exercise intervention and nutrition intervention.Biochemical indices,body weight and body mass index were compared between the two groups.Results Before chemotherapy and from the 2nd to the 4th cycle of chemotherapy,the white blood cell count in the control group were(6.41±2.69)×109/L,(6.28±3.44)×109/L,(5.23±1.20)×109/L,(5.21±1.27)×109/L,(4.06±2.36)×109/L;the absolute values of neutrophils were(4.39±2.30)×109/L,(4.30±2.05)×109/L,(4.10±0.33)×109/L,(3.77±0.46)×109/L,(3.36±1.28)×109/L;the absolute values of total lymphocytes were(1.42±0.66)×109/L,(1.34±0.72)×109/L,(1.00±0.68)×109/L,(1.00±0.46)×109/L,(0.98±0.33)×109/L;the platelet count were(221.22±90.93)×109/L,(215.98±90.23)×109/L,(202.39±21.12)×109/L,(192.60±32.76)×109/L,(187.44±15.69)×109/L;the hemoglobin were(121.20±25.16),(120.34±16.68),(112.23±15.12),(108.30±16.20),(103.98±14.25)g/L;the total protein were(65.33±4.35),(65.58±5.66),(60.65±6.75),(58.89±5.69),(57.34±6.23)g/L;the albumin were(44.36±5.24),(43.88±5.79),(39.03±6.75),(37.85±5.69),(26.31±6.28)g/L,respectively.In the study group,the white blood cell count were(6.20±2.36)×109/L,(6.88±5.64)×109/L,(5.95±1.45)×109/L,(5.79±1.56)×109/L,(5.63±4.36)×109/L;the absolute values of neutrophils were(4.35±2.56)×109/L,(4.86±5.36)×109/L,(4.30±0.56)×109/L,(4.12±0.34)×109/L,(3.90±1.30)×109/L;the absolute values of total lymphocytes were(1.44±0.86)×109/L,(1.46±0.77)×109/L,(1.33±0.68)×109/L,(1.26±0.77)×109/L,(1.14±0.42)×109/L;the platelet count were(224.12±94.23)×109/L,(222.20±82.59)×109/L,(220.19±33.31)×109/L,(205.43±25.33)×109/L,(197.20±24.26)×109/L;the hemoglobin were(123.55±23.95),(122.26±21.98),(119.24±15.23),(116.20±22.13),(114.01±20.46)g/L;the total protein were(65.12±4.76),(65.68±5.26),(63.45±5.06),(61.30±4.03),(60.11±4.82)g/L;the albumin were(43.25±6.69),(44.98±6.27),(41.65±6.03),(40.20±5.03),(39.25±6.03)g/L.There was no statistically significant difference between the two groups in terms of white blood cell count,absolute value of neutrophils,absolute value of total lymphocytes,platelet count,hemoglobin,total protein,albumin before and during the first cycle of chemotherapy(P>0.05).From the 2nd to the 4th cycle,the white blood cell count,absolute value of neutrophils,absolute value of total lymphocytes,platelet count,hemoglobin,total protein,albumin of the study group were better than those of the control group,with a statistically significant difference(P<0.05).Before chemotherapy and from the 1st to 4th cycles of chemotherapy,the body weight in the control group were(65.02±11.34),(65.29±11.26),(61.00±7.26),(59.03±9.36),(58.69±8.46)kg;the body mass index were(23.56±2.99),(23.87±2.11),(22.26±2.29),(21.16±2.59),(21.06±2.21)kg/m2.In the study group,the body weight were(65.30±10.58),(65.29±10.75),(64.55±10.06),(63.88±10.74),(62.56±9.20)kg;the body mass index were(23.56±2.96),(23.54±2.75),(23.41±2.48),(23.01±2.45),(22.12±2.05)kg/m2.There was no statistically significant difference in body weight and body mass index between the two groups before and during the first cycle of chemotherapy(P>0.05).From the 2nd to the 4th cycle of chemotherapy,the body weight and body mass index in the study group were better than those in the control group,with a statistically significant difference(P<0.05).Conclusion Nutritional intervention has certain clinical value on the nutritional level of patients with non-small cell lung cancer during chemotherapy,and is worthy of extensive promotion and application.
