Comparison of long-term effects of different irrigation and disinfection drugs on periodontal-endodontic combined lesions in root canal therapy
Objective To analyze the long-term effect of different irrigation and disinfection drugs on periodontal-endodontic combined lesions in root canal therapy.Methods 110 patients with periodontal-endodontic combined lesions were divided into a reference group and a study group using random numerical table,with 55 cases in each group.All patients were treated with root canal therapy.The reference group was assisted by ultrasonic irrigation with sodium hypochlorite solution and disinfection with calcium hydroxide,while the study group was assisted by ultrasonic irrigation with tinidazole solution and disinfection with minocycline hydrochloride.Both groups were compared in terms of periodontal plaque index(PLI),periodontal pocket depth(PD),probing bleeding index(BI),gingival index(GI)and clinical attachment level(CAL),curative effect,recurrence rate,incidence of tooth looseness,periodontitis and percussion pain.Results The total effective rate of the study group was 96.36%,which was higher than that of 85.45%of the reference group,and the difference was statistically significant(P<0.05).At 1 month and 3 months after treatment,the BI,PD,PLI,GI and CAL in the study group were(1.96±0.54),(4.48±0.46)mm,(1.29±0.56),(1.47±0.33),(3.40±0.47)mm and(1.62±0.40),(4.22±0.40)mm,(1.05±0.43),(0.80±0.25),(3.18±0.44)mm,which were significantly lower than those of(2.30±0.52),(4.90±0.53)mm,(1.68±0.49),(1.69±0.31),(3.83±0.52)mm and(1.93±0.47),(4.87±0.47)mm,(1.33±0.40),(1.23±0.27),(3.65±0.46)mm in the reference group.The difference was statistically significant(P<0.05).At 6 months after treatment,PD of(4.05±0.33)mm and CAL of(3.10±0.38)mm in the study group were significantly lower than those of(4.24±0.35)and(3.42±0.37)mm in the reference group,and the difference was statistically significant(P<0.05).At 6 months after treatment,there were no significant differences in BI,GI and PLI between the two groups(P>0.05).At 6 months after treatment,the recurrence rate in the study group was 3.64%,the incidence of tooth loosening was 1.82%,and the incidence of periodontitis was 5.45%,which were lower than those of 14.55%,12.73%,18.18%in the reference group,and the difference was statistically significant(P<0.05).There was no significant difference in the incidence of percussion pain between the two groups(P>0.05).Conclusion Tinidazole solution+minocycline is suitable irrigation and disinfection drug,which has the good long-term effect and can improve the periodontal condition of patients with periodontal-endodontic combined lesions and prevent the recurrence of the disease.
