Effect observation of small incision anatomical titanium plate internal fixation on displaced comminuted calcaneal fracture
Objective To observe the curative effect of small incision anatomical titanium plate internal fixation on comminuted displaced calcaneal fracture,so as to provide reference for clinical research. Methods A total of 82 patients with comminuted displaced calcaneal fracture were randomly divided into reference group and study group, with 41 cases in each group. The reference group was treated with traditional incision-locked plate internal fixation, and the study group was treated with small-incision anatomical titanium plate internal fixation.Comparison was made on visual analogue scale(VAS)score,Baird ankle score,ankle-hind foot scale(AOFAS)score, calcaneal physiological and anatomical structure indicators(calcaneus height, calcaneus width, axial length of calcaneus and Bohler angle),and surgical indicators(operative time, intraoperative blood loss, fracture healing time and hospitalization time),treatment effect and complications between the two groups. Results After operation, the VAS score of(2.31±0.93)points in the study group was lower than(3.87±0.78)points in the reference group;and the Baird ankle score of(94.95±2.82)points and AOFAS score of(88.41±2.70)points in the study group were higher than(90.44±2.73)and(84.49±2.47)points in the reference group(P<0.05).After operation, the calcaneus width of(29.73±1.25)mm and calcaneus height of(29.44±1.02)mm in the study group were lower than(39.38±1.76)and(39.47±0.96)mm in the reference group;and the axial length of calcaneus of(68.43±1.26)mm in the study group was longer than(54.32±3.77)mm in the reference group(P<0.05).The operative time of the study group was(68.37±6.83)min,the hospitalization time was(10.84±0.86)d and the fracture healing time was(72.13±2.54)d,which were shorter than(87.84±11.78)min,(14.78±0.51)d and(93.57±3.49)d of the reference group;the intraoperative blood loss of(103.64±13.72)ml of the study group was less than(152.79±11.63)ml of the reference group(P<0.05).The total effective rate of 95.12%in the study group was higher than that of 80.49%in the reference group(P<0.05).The complication rate of 2.44%in the study group was lower than 19.51%in the reference group(P<0.05).Conclusion For patients with comminuted displaced calcaneal fracture, small incision anatomical titanium plate internal fixation can not only save operative time, reduce intraoperative blood loss, but also improve ankle and foot function, relieve pain, prevent complications, and have a significant therapeutic effect.
