Study on the effect of percutaneous endoscopic interlaminar discectomy for lumbar disc herniation
Objective To explore the effect of percutaneous endoscopic interlaminar discectomy for lumbar disc herniation(LDH).Methods 60 patients with LDH were randomly divided into an intervention group and a control group, with 30 cases in each group. The control group underwent percutaneous transforaminal endoscopic discectomy, and the intervention group underwent percutaneous endoscopic interlaminar discectomy. The therapeutic effect, surgical indexes, visual analogue scale(VAS)score and Oswestry disability index(ODI)score, and the incidence of complications were compared between the two groups. Results The total effective rate of 96.67%in the intervention group was higher than 80.00%in the control group(P<0.05).In the control group, the intraoperative blood loss, operative time, intraoperative fluoroscopy times and hospitalization days were(68.35±12.36)ml,(73.16±10.58)min,(24.76±4.85)times and(5.42±1.36)d,respectively;in the intervention group, they were(50.36±10.22)ml,(46.39±10.34)min,(7.82±2.05)times and(5.23±1.08)d,respectively.The intraoperative blood loss, operative time and intraoperative fluoroscopy times in the intervention group were less than those in the control group(P<0.05).There was no significant difference in hospitalization days between the two groups(P>0.05).In the control group, the VAS scores before and after treatment were(7.89±1.05)and(1.85±0.62),and the ODI scores before and after treatment were(38.35±4.43)and(21.35±4.22),respectively.In the intervention group, the VAS scores before and after treatment were(8.02±1.11)and(1.59±0.66)points,and the ODI scores before and after treatment were(39.12±4.28)and(20.53±4.16)points.The VAS scores and ODI scores in both groups after treatment were lower than those before treatment(P<0.05).There was no significant difference in VAS score and ODI score between the two groups after treatment(P>0.05).There was no significant difference in complication rate between the two groups(P>0.05).Conclusion Percutaneous endoscopic interlaminar discectomy has significant efficacy on LDH patients, and can effectively improve the pain symptoms and lumbar function. The intraoperative blood loss is relatively small, the operation time is short. It is safe and reliable, and is conducive to postoperative rehabilitation, which is worthy of promotion and application.
