Application value of color Doppler ultrasound in preoperative and postoperative evaluation of autologous arteriovenous fistula in hemodialysis patients
Objective To investigate the application value of color Doppler ultrasound in the preoperative evaluation of autologous arteriovenous fistula(AVF)in hemodialysis patients.Methods A total of 60 patients with end-stage renal disease(ESRD)receiving AVF maintenance hemodialysis were selected for this study.Color Doppler ultrasound was used for preoperative evaluation and postoperative dynamic monitoring,and the success rate of surgery was evaluated after surgery.Patients were divided into a successful group(56 cases)and a failure group(4 cases)according to the results of the surgery.The clinical data(age,gender,primary disease,radial artery diameter,cephalic vein diameter and arterial resistance index)of the patients in the successful group and the failure group were compared.Results Postoperatively,48 and 12 of 60 hemodialysis patients underwent standard AVF of the left forearm and right forearm,respectively.After a 3-month follow-up and visit,56(93.33%)of the 60 patients had normal internal fistula fistulae and were suitable for hemodialysis treatment.4 cases(6.67%)showed failure of internal fistula surgery,with 2 cases of diabetic nephropathy and 2 cases of glomerulonephropathy,3 of which were characterized by poor anastomotic maturation and 1 other case of anastomotic failure.There were no significant differences in gender,age,radial artery diameter,primary disease and arterial resistance index between the successful group and the failure group(P>0.05).The cephalic vein diameter of the successful group was(1.763±0.572)mm,and that of the failure group was(1.000±0.469)mm.The difference between the two groups was statistically significant(P<0.05).Conclusion Preoperative color Doppler ultrasound evaluation of AVF is of great significance.Color Doppler ultrasound can real-time dynamic observation of vascular anatomy and functional information,which can be more accurate guidance of end-stage renal disease fistula patients vascular selection,and improve the success rate of surgery.
