Analysis of the treatment effect of high-intensity focused ultrasound combined with mifepristone for uterine scar pregnancy
Objective To study the clinical efficacy of high-intensity focused ultrasound combined with mifepristone for uterine scar pregnancy.Methods 64 patients with uterine scar pregnancy were divided into a reference group and an experimental group by parity grouping method.The reference group was treated with mifepristone,while the experimental group was treated with mifepristone and high-intensity focused ultrasound therapy.Both groups were compared in terms of treatment effect,prognostic effect[β-human chorionic gonadotropin(β-HCG)recovery time,menstrual recovery time,length of hospital stay],experimental examination[β-HCG,hypersensitive C-reactive protein(hs-CRP),interleukin(IL)-8,IL-17]results,ultrasonography[mass volume,blood flow index(FI),pulsation index(PI),vascularization index(VI)]results,and clinical related indexes(vaginal bleeding time,bleeding volume in uterus curettage,pain score before and after treatment).Results The total clinical effective rate of the experimental group was 96.88%,which was higher than 81.25%of the reference group(P<0.05).In the experimental group,the β-HCG recovery time was(3.02±0.36)d,the menstrual recovery time was(1.39±0.42)months and the length of hospital stay was(6.75±1.25)d,which were shorter than(4.12±1.12)d,(2.53±0.61)months and(13.26±3.41)d in the reference group(P<0.05).After treatment,the levels of β-HCG,hs-CRP,IL-8 and IL-17 in the experimental groups were(1389.22±167.43)U/L,(9.48±2.15)mg/L,(66.13±6.20)pg/ml and(8.15±2.53)pg/ml,and those in the reference groups were(1873.54±216.40)U/L,(12.94±2.20)mg/L,(73.96±6.59)pg/ml and(11.95±3.26)pg/ml.The levels of β-HCG,hs-CRP,IL-8 and IL-17 in the experimental group were lower than those in the reference group(P<0.05).After treatment,the mass volume,FI,PI and VI of the experimental groups were(1.97±0.25)cm3,(0.05±0.01),(0.59±0.10)and(6.07±0.86),and those of the reference group were(2.46±0.32)cm3,(0.08±0.02),(0.79±0.14)and(8.57±0.92).The mass volume,FI,PI and VI of the experimental group were lower than those of the reference group(P<0.05).Before treatment,there was little difference in pain scores between the two groups(P>0.05).In the experimental group,the vaginal bleeding time of(2.58±0.29)d was shorter than(5.07±1.31)d in the reference group,the bleeding volume in uterus curettage of(45.12±4.62)ml was less than(258.06±24.82)ml in the reference group,and the pain score of(2.58±0.25)points after treatment was lower than(4.31±0.32)points in the reference group(P<0.05).Conclusion In the treatment of uterine scar pregnancy,the combined use of high-intensity focused ultrasound and mifepristone has significant clinical value.On the one hand,the patient's treatment effect is improved,and the recovery time of β-HCG is shortened.On the other hand,the patient's laboratory indicators and the ultrasonographic indicators recovere faster,and the pain is lessened.
