Exploration of the correlation between miR-891a-5p and miR-383-5p and the development of distant metastasis after surgery for early-stage hormone receptor-positive breast cancer
Objective To explore the correlation between micro RNAs(miRNAs,miR)-891a-5p and miR-383-5p and the development of distant metastasis after surgery for early-stage hormone receptor(HR)-positive breast cancer.Methods A total of 62 cases of early-stage(stage T1~2N0M0)HR-positive breast cancer patients were divided into a distant metastasis group(28 cases with distant metastasis)and a non-distant metastasis group(34 cases with no distant metastasis)according to whether there was distant metastasis after surgery.Clinical breast cancer specimens from the two groups were collected and the expression levels of miRNAs in the tissues were detected.The expression levels of miR-891a-5p,miR-383-5p,and miR-1295a were compared between the two groups to analyze the diagnostic efficiency of miR-891a-5p,miR-383-5p,and miR-1295a on distant metastasis of breast cancer.The expression levels of miR-661,miR-296-3p,and miR-128-3p in the two groups were compared,and the correlation of expression levels of miR-891a-5p,miR-383-5p,and miR-1295a with the distant metastasis-free survival(DMFS).Results The expression levels of miR-891a-5p,miR-383-5p,and miR-1295a in the distant metastasis group were significantly reduced compared with those in the non-distant metastasis group,and the multiplicity of difference were-2.519,-1.812,and-1.443.The expression of miR-891a-5p in the distant metastasis group was(1.099±0.899),which was significantly lower than(2.768±2.194)in the non-distant metastasis group(P<0.05).The sensitivity and specificity of low miR-891a-5p expression(<1.12)in predicting distant metastasis were 71.4%and 82.4%,with an area under the curve(AUC)of 0.825(P=0.0000<0.01).The sensitivity and specificity of miR-383-5p low expression(<1.15)in predicting distant metastasis were 53.4%and 85.3%,with an AUC of 0.738(P=0.0013<0.01).In contrast,the sensitivity and specificity of miR-1295a low expression(<2.08)in predicting distant metastasis were 82.1%and 50.0%,with an AUC of only 0.677(P=0.0175<0.05).In terms of diagnostic efficiency,the AUC,sensitivity,positive predictive value,and negative predictive value of miR-891a-5p in diagnosing distant metastasis of breast cancer were better than that of miR-383-5p,and the diagnostic efficacy of miR-1295a was the worst,which indicated that the low expression of miR-891a-5p and miR-383-5p could predict distant metastasis of early-stage HR-positive breast cancer.The expression levels of miR-661,miR-296-3p,and miR-128-3p in the distant metastasis group were lower than those in the non-distant metastasis group,and the difference was statistically significant(P<0.05).The follow-up time of 62 patients ranged from 59 to 131 months,and the median follow-up time was 81 months.The median DMFS of patients with low miR-891a-5p expression was 48 months,and the 5-year DMFS was only 38.5%,which was lower than 86.1%of patients with high miR-891a-5p expression(P<0.01).The median DMFS time of patients with low miR-383-5p expression was 52 months,and the 5-year DMFS was 35.0%,which was lower than 73.8%of patients with high miR-383-5p expression(P<0.01).The median DMFS in patients with low miR-1295a expression was 65 months,and there was no significant difference in 5-year DMFS between patients with high and low miR-1295a expression(P>0.05).Conclusion miR-891a-5p and miR-383-5p have the potential to be a valuable prognostic biomarker and tumor target for HR-positive breast cancer.
