支气管扩张急性加重期合并2型糖尿病患者肺功能的变化特点探讨

目的 探讨支气管扩张急性加重期合并2型糖尿病(T2DM)的肺功能变化特点.方法 74例支气管扩张急性加重期患者,将其中 23 例明确诊断为单纯支气管扩张急性加重期患者作为A组,25 例明确诊断为支气管扩张急性加重期合并T2DM且血糖控制良好患者作为B组,26 例明确诊断为支气管扩张急性加重期合并T2DM但血糖控制不良患者作为C组.对三组患者进行肺功能检查,比较三组患者用力肺活量占预计值的百分比(FVC%)、第 1 秒用力呼气容积占用力肺活量的百分比(FEV1/FVC).结果 A组FVC%为(82.4504±11.4826)%,FEV1/FVC 为(81.5704±12.0518)%;B组FVC%为(74.6760±12.5300)%,FEV1/FVC 为(74.0324±11.7070)%;C 组 FVC%为(67.0315±14.0777)%,FEV1/FVC为(66.9266±11.8841)%.A组FVC%高于B组(P<0.05);A组FVC%高于C组(P<0.01);B组FVC%高于C组(P<0.05).A组FEV1/FVC高于B组(P<0.05);A组FEV1/FVC高于C组(P<0.01);B组FEV1/FVC高于C组(P<0.05).结论 T2DM对支气管扩张急性加重期患者的肺功能有明显影响,血糖控制不良加剧了该类患者的肺功能下降.
Investigation on the characteristics of pulmonary function in patients with acute exacerbation of bronchiectasis complicated with type 2 diabetes mellitus
Objective To explore the characteristics of pulmonary function in patients with acute exacerbation of bronchiectasis complicated with type 2 diabetes mellitus(T2DM).Methods Among 74 patients with acute exacerbation of bronchiectasis,23 patients with acute exacerbation of bronchiectasis alone were selected as group A,25 patients with acute exacerbation of bronchiectasis combined with T2DM and well-controlled blood glucose as group B,26 patients with acute exacerbation of bronchiectasis combined with T2DM and poor-controlled blood glucose as group C.The pulmonary function examination was performed on the three groups of patients,and forced vital capacity as a percentage of the predicted value(FVC%)and the ratio of forced expiratory volume in one second to forced vital capacity(FEV1/FVC)of the three groups of patients were compared.Results In group A,FVC%was(82.4504±11.4826)%,FEV1/FVC was(81.5704±12.0518)%;in group B,FVC%was(74.6760±12.5300)%,FEV1/FVC was(74.0324±11.7070)%;in group C,FVC%was(67.0315±14.0777)%and FEV1/FVC was(66.9266±11.8841)%.FVC%of group A was higher than that of group B(P<0.05).FVC%of group A was higher than that of group C(P<0.01).FVC%of group B was higher than that of group C(P<0.05).FEV1/FVC in group A was higher than that in group B(P<0.05).FEV1/FVC in group A was higher than that in group C(P<0.01).FEV1/FVC in group B was higher than that in group C(P<0.05).Conclusion Patients with T2DM have a significant impact on the pulmonary function of patients with acute exacerbation of bronchiectasis,and poor glycemic control aggravates the decline of pulmonary function in these patients.
