Study on risk factors and treatment countermeasures of infection after percutaneous nephrolithotomy in patients with kidney stones
Objective To explore the risk factors and treatment countermeasures of infection after percutaneous nephrolithotomy(PCNL)in patients with kidney stones.Methods In this study,100 patients with kidney stones were treated by percutaneous nephrolithotomy and divided into systemic inflammatory response syndrome(SIRS)group(41 cases)and non-SIRS group(59 cases)according to whether they developed SIRS after surgery.The general data of the two groups were recorded during perioperative period,and the risk factors of postoperative infection were analyzed.Results There were significant differences in percutaneous nephroscope channel,stone load,intraoperative stone bacteria culture and preoperative urinary bacteria culture in midstream urine between non-SIRS group and SIRS group(P<0.05).There were no significant differences in gender,stone type,comorbidities and intraoperative perfusion pressure between the two groups(P>0.05).Logistic multivariate regression analysis showed that large stone load[OR=2.162;95%CI=(1.319,3.543);P=0.002<0.05],percutaneous nephroscope multichannel[OR=1.225;95%CI=(1.029,1.458);P=0.022<0.05],positive for intraoperative stone bacteria culture[OR=2.351;95%CI=(1.069,5.170);P=0.033<0.05],positive for preoperative urinary bacteria culture in midstream urine[OR=2.344;95%CI=(1.176,4.674);P=0.033<0.05]were risk factors for infection after percutaneous nephrolithotomy in patients with kidney stones.Conclusion The risk factors for infection after percutaneous nephrolithotomy in patients with kidney stones include large stone load,multichannel,positive for intraoperative stone bacteria culture,and positive for preoperative urinary bacteria culture in midstream urine.It is very important to carry out timely treatment countermeasures after analyzing relevant factors,which can effectively prevent postoperative infection.
