Analysis of the outcome of trial of labor for women with scarred uterus due to cesarean section
Objective To analyze the outcome and safety of trial of labor for women with scarred uterus due to cesarean section.Methods There were 160 women with scarred uterus due to cesarean section,of which 80 women with scarred uterus who underwent trial of labor after cesarean(TOLAC)were included in the observation group,and 80 women with scarred uterus undergoing elective repeated cesarean section(ERCS)were included in the control group.Failure of TOLAC and its causes in the observation group were analyzed,and the length of hospital stay,total blood loss during delivery and 24 h postpartum,pregnancy outcome and perinatal outcome were compared between the two groups.Results In the observation group,the success rate of TOLAC was 85.00%(68/80),and the failure rate was 15.00%(12/80),and the reasons for failure included abnormal labor,maternal fear of painful contractions,fetal distress,and incomplete uterine rupture.The observation group had shorter length of hospital stay of(3.12±1.01)d than(5.36±1.36)d in the control group,and less total blood loss of(245.36±19.95)ml during delivery and 24 h postpartum than(417.53±21.18)ml in the control group(P<0.05).The incidence of amniotic fluid contamination,postpartum hemorrhage,blood transfusion,uterine rupture,hysterectomy and bladder damage in the two groups were compared,and the difference was small(P>0.05).The observation group had lower neonatal birth weight of(3335.29±163.25)g than(3526.95±155.96)g in the control group(P<0.05).There were small difference in Apgar score,incidence of neonatal asphyxia,incidence of hypoxic-ischemic encephalopathy between the two groups(P>0.05).Conclusion TOLAC is safe and feasible for women with scarred uterus due to cesarean section who have trial of labor indication after comprehensive prenatal evaluation and close monitoring during labor.
