Effect of combined treatment with α-lipoic acid and alprostadil on micro-angiogenesis,oxidative stress and wound healing in patients with diabetic foot
Objective To explore the effect of combined treatment with α-lipoic acid and alprostadil on micro-angiogenesis,oxidative stress and wound healing in patients with diabetic foot(DF).Methods Using the random number table method,98 patients with DF were divided into the combination group(49 cases)and the control group(49 cases).The control group was treated with alprostadil,while the combination group was treated with α-lipoic acid combined with alprostadil.The clinical efficacy,micro-angiogenesis,oxidative stress response,and wound healing were compared between the two groups.Results The total clinical effective rate of 95.91%in the combination group was significantly higher than 81.63%in the control group(P<0.05).After 1 month of treatment,the level of vascular growth endothelial factor(VEGF)in both groups was significantly higher than that before treatment,and the level of endostatin(ES)was significantly lower than that before treatment;the combination group had significantly higher VEGF of(135.28±13.58)ng/L than(103.29±12.39)ng/L in the control group,and significantly lower ES of(41.06±4.59)ng/ml than(49.68±5.12)ng/ml in the control group(P<0.05).After 1 month of treatment,the level of superoxide dismutase(SOD)in both groups was significantly higher than that before treatment,and the level of malondialdehyde(MDA)was significantly lower than that before treatment;SOD of(92.77±10.25)U/ml in the combination group was significantly higher than(86.29±9.21)U/ml in the control group;MDA of(2.29±0.94)μmol/L in the combination group was significantly lower than(2.97±1.06)μmol/L in the control group(P<0.05).The wound healing time of(16.28±3.12)d in the combination group was significantly shorter than(21.58±4.05)d in the control group,and the wound healing rate of(76.29±10.39)%was significantly higher than(54.95±11.28)%in the control group(P<0.05).After 1 month of treatment,the wound swelling score in both groups were lower than that before treatment,and the wound swelling score of(0.93±0.34)points in the combination group was significantly lower than(1.17±0.41)points in the control group(P<0.05).Conclusion Compared to monotherapy with alprostadil,the combined treatment with α-lipoic acid and alprostadil is more effective in treating patients with DF.The latter can promote micro-angiogenesis,improve oxidative stress response,and promote wound healing.
