摘要
目的 探讨康肾方治疗慢性肾衰竭的临床疗效,并总结临证经验.方法 67例慢性肾衰竭患者,采用奇偶数法将其分为对照组(35 例)和观察组(32 例).对照组患者进行西药常规治疗,观察组在对照组基础上加用康肾方治疗.对比两组治疗前后血肌酐(Scr)、内生肌酐清除率(Ccr)、血尿素氮(BUN)、血清转化生长因子-β1(TGF-β1)、血红蛋白(Hb)、尿蛋白、血清白蛋白(ALB)、中医证候积分及疗效、安全性.结果 观察组治疗后Scr(110.0±5.2)μmol/L、BUN(6.6±0.2)mmol/L、TGF-β1(2.0±0.1)ng/ml和中医证候积分(4.83±1.91)分低于对照组的(138.0±7.2)μmol/L、(8.7±0.3)mmol/L、(3.1±0.1)ng/ml、(8.77±1.85)分,Ccr(71.0±0.8)ml/min高于对照组的(55.4±0.7)ml/min(P＜0.05);观察组治疗总有效率93.75%高于对照组的 71.43%(P＜0.05).两组治疗后Hb、尿蛋白、ALB对比无明显差异(P＞0.05).两组患者在治疗过程中均未出现不良反应.结论 康肾方是以"固肾健脾法"为治疗思路的经验方,对脾肾气虚型慢性肾衰竭有效.
Abstract
Objective To explore the clinical efficacy of Kangshen recipe in treating chronic renal failure,and summarize its clinical experience.Methods 67 patients with chronic renal failure were divided into a control group(35 cases)and an observation group(32 cases)by odd and even number method.The control group received conventional western medicine treatment,and the observation group was treated with Kangshen recipe on the basis of the control group.Both groups were compared in termsof serum creatinine(Scr),creatinine clearance rate(Ccr),blood urea nitrogen(BUN),serum transforming growth factor-β1(TGF-β1),hemoglobin(Hb),urinary protein,serum albumin(ALB)and traditional Chinese medicine syndrome score before and after treatment,efficacy,and safety.Results After treatment,the observation group had Scr of(110.0±5.2)μmol/L,BUN of(6.6±0.2)mmol/L,TGF-β1 of(2.0±0.1)ng/ml and traditional Chinese medicine syndrome score of(4.83±1.91)points,which were lower than(138.0±7.2)μmol/L,(8.7±0.3)mmol/L,(3.1±0.1)ng/ml and(8.77±1.85)points in the control group;the observation group had higher Ccr of(71.0±0.8)ml/min than(55.4±0.7)ml/min in the control group(P＜0.05).After treatment,the total effective rate of 93.75%in the observation group was higher than 71.43%in the control group(P＜0.05).After treatment,there were no significant differences in Hb,urinary protein and ALB between the two groups(P＞0.05).No adverse reactions occurred during treatment in both groups.Conclusion Kangshen recipe is an empirical prescription based on"strengthening the kidney and the spleen",which is effective for chronic renal failure with spleen-kidney qi deficiency.
