Study on clinical efficacy of combining dopamine,furosemide and nitroglycerin in patients with rheumatic heart disease and heart failure
Objective To analyze the effect of combining dopamine,furosemide and nitroglycerin in patients with rheumatic heart disease and heart failure.Methods 140 patients with rheumatic heart disease and heart failure were divided into a control group and an observation group according to random number table method,each with 70 cases.The control group was treated with dopamine,and the observation group was treated with dopamine combined with furosemide and nitroglycerin.The clinical efficacy,cardiac function index,incidence of adverse reactions,brain natriuretic peptide level and 6-min walking distance before and after treatment were compared between the two groups.Results The total effective rate of the observation group was 95.71%,which was higher than the control group's 81.43%(P<0.05).Before treatment,there was no differences in left ventricular ejection fraction(LVEF),left ventricular end-systolic diameter(LVESD)and left ventricular end-diastolic diameter(LVEDD)between the two groups(P>0.05).After treatment,LVEF was significantly increased,and LVEDD and LVESD were significantly decreased in both groups;the observation group had LVEF of(39.38±3.76)%,LVEDD of(61.02±5.92)mm and LVESD of(45.47±4.29)mm,which were significantly better than(35.24±3.31)%,(66.39±6.48)mm and(51.03±4.88)mm in the control group(P<0.05).The incidence of adverse reactions in the observation group was 10.00%,which was not different from 7.14%in the control group(P>0.05).Before treatment,there was no difference in brain natriuretic peptide level and 6-min walking distance between the two groups(P>0.05).After treatment,the brain natriuretic peptide level and 6-min walking distance in both groups were better than those before treatment;the observation group had brain natriuretic peptide of(762.72±58.54)ng/L and 6-min walking distance of(363.24±25.27)m,which were better than(953.48±71.06)ng/L and(310.27±22.31)m in the control group(P<0.05).Conclusion Combining dopamine and furosemide and nitroglycerin has a high therapeutic effect on patients with rheumatic heart disease and heart failure,which can significantly improve their heart function,lower the level of brain natriuretic peptide,and improve the physical ability,and there are no obvious adverse reactions during the treatment.
