Analysis of factors affecting central lymph node metastasis in thyroid micropapillary carcinoma and the significance of clearance
Objective To analyze the factors affecting central lymph node metastasis in thyroid micropapillary carcinoma and the significance of clearance.Methods The clinical data of 597 cases of thyroid micropapillary carcinoma were retrospectively analyzed,and the gender,age,tumor diameter,whether it was multifocal,whether it invaded the capsule,and whether it was complicated with Hashimoto's thyroiditis and central lymph node metastasis were counted.The factors affecting central lymph node metastasis in thyroid micropapillary carcinoma was analyzed.Results All 597 patients underwent centralized lymph node dissection on the diseased side,including 161 cases with metastasis,accounting for 26.97%,and 436 cases without metastasis,accounting for 73.03%.Among them,236 cases were≤45 years of age,accounting for 39.53%;361 cases were>45 years of age,accounting for 60.47%.The tumor diameter was≤0.5 cm in 322 cases,accounting for 53.94%;>0.5 cm in 275 cases,accounting for 46.06%.Univariate analysis showed that 57.14%of metastatic patients were≤45 years of age,22.36%were male,62.73%had a tumor diameter>0.5 cm,56.52%had capsule invasion,and 38.51%were multifocal,which were higher than 33.03%,13.76%,39.91%,42.89%,and 22.48%in patients without metastasis,and the difference was statistically significant(P<0.05).There was no significant difference in Hashimoto thyroiditis between patients without metastasis and those with metastasis(P>0.05).Multivariate Logistic regression analysis showed that male,≤45 years of age,tumor diameter>0.5 cm,and multifocal were independent risk factors for central lymph node metastasis in papillary thyroid microcarcinoma(P<0.05).Conclusion There is no correlation between Hashimoto's thyroiditis and central lymph node metastasis.Male,≤45 years of age,tumor diameter>0.5 cm,multifocal,and capsule invasion are independent risk factors for central lymph node metastasis in thyroid micropapillary carcinoma.
