Study on clinical effect of Carboprost in prevention of postpartum hemorrhage
Objective To study the clinical effect of carboprost methylate suppository(trade name:Carboprost)in prevention of postpartum hemorrhage.Methods 108 cases of postpartum hemorrhage patients were divided into a control group and an observation group according to digital random table method,each with 54 cases.The control group was given oxytocin to prevent postpartum hemorrhage,and the observation group was given Carboprost to prevent postpartum hemorrhage.The postpartum hemorrhage,clinical efficacy,occurrence of adverse reactions,coagulation function and inflammatory factors before and after treatment were compared between the two groups.Results In the observation group,the postpartum hemorrhage rate was 7.4%(4/54),and the amount of hemorrhage were(215.13±27.20)and(234.62±29.41)ml at 2 and 24 h postpartum;in the control group,the postpartum hemorrhage rate was 29.6%(16/54),and the amount of hemorrhage were(336.32±38.08)and(380.17±42.26)ml at 2 and 24 h postpartum.There were significant differences in the rate of postpartum hemorrhage and amount of hemorrhage at 2 and 24 h postpartum between the two groups(P<0.05).After treatment,the observation group had fibrinogen(FIB)of(4.69±0.13)g/L,activated partial thromboplastin time(APTT)of(30.06±1.88)s,prothrombin time(PT)of(12.06±0.30)s,and D-dimer(D-D)of(0.37±0.22)mg/L,and the difference was significant when compared with(4.09±0.15)g/L,(32.23±2.10)s,(13.55±0.42)s,and(0.63±0.29)mg/L in the control group(P<0.05).After treatment,CRP,IL-8 and TNF-α in the control group were(37.82±5.02)mg/L,(34.92±6.27)pg/ml and(28.62±5.71)pg/ml,and the difference was significant when compared with(20.52±4.51)mg/L,(23.02±6.14)pg/ml and(20.16±4.27)pg/ml in the observation group(P<0.05).In the observation group,33 cases were obviously effective,accounting for 61.1%,20 cases were effective,accounting for 37.0%,1 case was ineffective,accounting for 1.9%,and the total effective rate was 98.1%.In the control group,16 cases were obviously effective,accounting for 29.6%,27 cases were effective,accounting for 50.0%,11 cases were ineffective,accounting for 20.4%,the total effective rate was 79.6%.The total effective rate of the observation group was significantly higher than that of the control group(x2=9.375,P<0.05).The observation group had 1 case of chest tightness and shortness of breath,1 case of elevated blood pressure,1 case of flushing,totaling 3 cases,accounting for 5.56%.The control group had 1 case of chest tightness and shortness of breath,1 case of elevated blood pressure,1 case of flushing,1 case of nausea and vomiting,totaling 4 cases,accounting for 7.41%.There was no significant difference in the incidence of adverse reactions between the two groups(x2=0.153,P>0.05).Conclusion Compared with oxytocin monotherapy,the combination of oxytocin and Carboprost has achieved significant efficacy in the treatment of postpartum hemorrhage,which can be used for reference.
