Study on correlation of the neutrophil/lymphocyte ratio with blood glucose level and complications in patients with type 2 diabetes mellitus
Objective To study the correlation of the neutrophil/lymphocyte ratio(NLR)with blood glucose level and complications in patients with type 2 diabetes mellitus(T2DM).Methods A total of 125 T2DM patients were studied,of which 50 T2DM patients were included in the T2DM group,75 cases of T2DM patients with complications(including coronary heart disease,diabetes nephropathy,retinopathy,etc.)were included in the T2DMC group;30 healthy subjects were included in the control group.(i)the baseline data[gender,age,body mass index(BMI),disease course,white blood cell count(WBC),systolic blood pressure(SBP),diastolic blood pressure(DBP),total cholesterol(TC),triglycerides(TG),hypersensitive C-reactive protein(hs CRP),neutrophil count,lymphocyte count],blood glucose indicators[fasting plasma glucose(FPG),glycated hemoglobin(HbA1c)]and NLR of T2DM group,TD2MC group and control group were compared.(ii)according to NLR,the 125 T2DM patients were divided into low NLR group(NLR≤2.35,45 cases)and high NLR group(NLR>2.35,80 cases).The blood glucose level and complication rate of the two groups were compared at admission,3 months after treatment.(iii)the correlation of NLR with blood glucose level and complications in T2DM patients was analyzed.Results There were significant differences in WBC,TC,TG,FPG,HbA1c,hs-CRP,neutrophil count,lymphocyte count and NLR among the three groups(P<0.05).WBC,TC,TG,FPG,HbA1c,hs-CRP,neutrophil count and NLR in T2DM group and T2DMC group were higher than those in the control group,while the lymphocyte count was lower than that in the control group;the disease course,WBC,TC,TG,FPG,HbA1c,hs-CRP,neutrophil count and NLR in T2DMC group were higher than those in T2DM group,and the lymphocyte count was lower than that in T2DM group;there was statistical significance(P<0.05).Among 125 T2DM patients,there were 45 cases with NLR≤2.35 and 80 cases with NLR>2.35 at admission.After treatment,FPG and HbA1c decreased in both groups compared with those at admission(P<0.05).In high NLR group,FPG were(9.70±0.98)and(7.48±0.79)mmol/L at admission and 3 months after treatment,and HbA1c were(8.33±0.85),(7.46±0.74)%,which were higher than(9.25±0.96),(7.15±0.74)mmol/L and(8.00±0.84),(7.13±0.75)%in low NLR group;hs-CRP was(2.98±0.34)mg/L at admission,Gensini atherosclerosis score was(60.13±6.45)points and complication rate was 67.50%,which were significantly higher than(2.46±0.25)mg/L,(43.12±4.39)and 46.67%in low NLR group(P<0.05).Correlation analysis showed that NLR in T2DM patients was positively correlated with FPG and HbA1c at admission,and also positively correlated with FPG,HbA1c and complication rate 3 months after treatment(P<0.05).Conclusion The NLR of patients with T2DM is closely related to the blood glucose level and the occurrence of complications.The high NLR indicates poor blood glucose control and high risk of complications,which can be used as a good predictor.
