Exploration of the diagnostic value of dual-energy CT for gout
Objective To explore the diagnostic value of dual-energy CT (DECT) for gout. Methods 96 patients with gout were selected as the observation group,and another 18 non-gout patients were selected as the control group. 96 gout patients were divided into the tophus group (16 cases) and the non-tophus group (80 cases) according to the presence or absence of tophus;the 96 gout patients were divided into the early stage (≤12 months,17 cases),the middle stage (13-36 months,12 cases) and the late stage (>36 months,67 cases) according to the duration of disease;and the gout patients were divided into the normal uric acid group (uric acid ≤420 μmol/L,29 cases) and the high uric acid group (uric acid>420 μmol/L,67 cases) according to the level of blood uric acid. All patients underwent DECT examination. The detection rates of urate crystal deposition were compared between the observation group and the control group,patients with different duration of disease,patients with different levels of uric acid,and patients with or without tophus. Results The detection rates of urate crystal deposition was 75.00% in the osbservation group,which was higher than 5.56% in the control group,and the difference was statistically significant (P<0.05). The detection rates of urate crystal deposition in patients with early,middle and late stages were 52.94% (9/17),58.33% (7/12) and 83.58% (56/67),respectively,and the difference was statistically significant (P<0.05),with late stage>middle stage>early stage. The detection rate of urate crystal deposition in the high uric acid group was 76.12%,which was higher than 72.41% in the normal uric acid group,but the difference was not statistically significant (P>0.05). The detection rate of urate crystal deposition in the tophus group was 100.00% higher than 70.00% in the non-tophus group,but the difference was statistically significant (P<0.05). Conclusion DECT has significant diagnostic value for gout. It can be used as one of the methods to diagnose and evaluate the condition of early gout and patients with tophus,and it has important value to guide the treatment and efficacy evaluation of gout.
