Effect of transurethral ureteroscopic holmium laser lithotripsy on ureteral calculi
Objective To explore the effect of transurethral ureteroscopic holmium laser lithotripsy on ureteral calculi.Methods 62 patients with ureteral calculi were selected and divided into holmium laser lithotripsy group and shock wave lithotripsy group according to different treatment methods,each with 31 cases.The holmium laser lithotripsy group was given transurethral ureteroscopic holmium laser lithotripsy,and the shock wave lithotripsy group was given extracorporeal shock wave lithotripsy.The perioperative indicators,lithotripsy effect,clinical efficacy,inflammatory stress indicators,traumatic stress indicators,renal function indicators and postoperative complications were compared between the two groups.Results The intraoperative blood loss of(10.86±1.45)ml in holmium laser lithotripsy group was less than(13.40±1.22)ml in shock wave lithotripsy group;in holmium laser lithotripsy group,the operation time was(61.26±5.90)min,the intestinal function recovery time was(13.25±2.17)h,the hematuria time was(2.73±0.40)d,the ambulation time was(3.18±1.07)d,and the hospitalization time was(5.03±1.01)d,which were shorter than(70.57±6.03)min,(17.48±2.34)h,(4.54±1.60)d,(4.74±1.15)d,and(6.40±1.32)d in shock wave lithotripsy group(P<0.05).In holmium laser lithotripsy group,the success rate of lithotripsy was 93.55%and the stone clearance rate was 96.77%,which were higher than 74.19%and 80.65%in shock wave lithotripsy group;the percentage of postoperative painkillers was 16.13%,which was lower than 51.61%in shock wave lithotripsy group(P<0.05).The total effective rate of 93.55%in holmium laser lithotripsy group was higher than 70.97%in shock wave lithotripsy group(P<0.05).After surgery,the levels of tumor necrosis factor-α(TNF-α),interleukin-1β(IL-1β),interleukin-6(IL-6),norepinephrine(NE),aldosterone(ALD),and cortisol(COR)in both groups were higher than those before surgery,and the levels of blood urea nitrogen(BUN),and serum creatinine(SCr)were lower than those before surgery(P<0.05).However,the differences in the levels of TNF-α,IL-1β,IL-6,NE,ALD,and COR between the two groups were not significant(P>0.05),and the levels of BUN and SCr in holmium laser lithotripsy group were lower than those in shock wave lithotripsy group(P<0.05).The postoperative complication rate of 6.45%in holmium laser lithotripsy group was lower than 29.03%in shock wave lithotripsy group(P<0.05).Conclusion Transurethral ureteroscopic holmium laser lithotripsy is effective in the treatment of ureteral calculi.
