Objective To explore the diagnostic value of three-dimensional echocardiography combined with electro-cardiogram(ECG)in the cardiac function impairment of patients with pulmonary arterial hypertension(PAH).Methods 178 patients with PAH admitted to our hospital from April 2021 to May 2022 were selected,among which 43 had cardiac function impairment and 135 did not.The value of 3D echocardiographic parameters and ECG parameters in the diagnosis of cardiac function damage caused by PAH was analyzed by receiver operating characteristic curve(ROC).Re-sults The values of right ventricular ejection fraction(RVEF)and tricuspid ring systolic displacement(TAPSE)in the occurrence group were lower than those in the non-occurrence group,and the right ventricular systolic blood pressure(RVSP)was higher than those in the non-occurrence group(P<0.05).There were no statistically significant differ-ences in heart rate(HR),corrected QT interval(QTc),QRS electrical axis,V1 lead R amplitude and V1 lead S ampli-tude between the two groups(P>0.05),while QRS duration and V5 lead S amplitude in the occurrence group were higher than those in the non-occurrence group(P<0.05).ROC curve showed that the area under the curve(AUC)of RVEF,RVSP and TAPSE in the diagnosis of cardiac function impairment in PAH alone was 0.686,0.701 and 0.735,respectively,and the AUC of the combined diagnosis was 0.760.The AUC of ECG parameters QRS duration and V5 lead S amplitude for diagnosis of cardiac function impairment in PAH alone were 0.730 and 0.701,respectively,and the AUC of combined diagnosis was 0.835.The sensitivity,specificity and AUC(0.95CI)of 3D echocardiography and ECG in the diagnosis of cardiac function impairment in PAH patients were 97.67%,69.63%and 0.889(0.857~0.946),re-spectively,which were higher than any other parameter,and the combined diagnosis efficiency of the two methods was higher(P<O.05).Conclusion Both three-dimensional echocardiography and ECG are valuable in the diagnosis of car-diac function impairment in PAH,and the combined diagnosis of the two methods is more valuable.
