高频超声联合25-(OH)D3、FGF23对2型糖尿病患者周围血管病变的预测价值

摘要
目的 探讨高频超声联合25羟维生素D3[25-(OH)D3]、成纤维细胞生长因子23(FGF23)对2型糖尿病(T2DM)患者周围血管病变(PVD)的预测价值.方法 选取2020年3月至2022年8月在华北医疗健康集团峰峰总医院治疗的T2DM合并PVD患者为病变组(n=95)、T2DM未合并PVD患者为未病变组(n=95),另选择同期行健康体检的健康者95例作为对照组.采用电化学发光法、ELISA法分别检测血清25-(OH)D3、FGF23表达水平;对T2DM患者行高频超声检查,比较病变组和未并病变组高频超声参数;ROC曲线分析血清25-(OH)D3、FGF23表达水平对T2DM患者合并PVD的预测效能,四表格法分析高频超声联合血清25-(OH)D3、FGF23对T2DM患者合并PVD的诊断价值.结果 对照组、未病变组、病变组血清25-(OH)D3表达水平依次降低,血清FGF23表达水平依次升高(P<0.05);血清25-(OH)D3、FGF23预测T2DM患者合并PVD的曲线下面积(AUC)分别为0.743、0.766,最佳截断值分别为24.81 μg/L、54.86 μg/mL;病变组IMT、斑块面积、RI均高于未病变组,PI、足背动脉血管腔直径、PSV均低于未病变组(P<0.05);高频超声联合血清25-(OH)D3、FGF23诊断T2DM患者合并PVD的灵敏度为96.84%,高于三者单独诊断(P<0.05).结论 T2DM合并PVD患者血清25-(OH)D3水平降低,FGF23水平升高,二者联合高频超声能够较好的预测T2DM患者PVD的发生.
Abstract
Objective To investigate the predictive value of high-frequency ultrasound combined with 25 hydroxyvi-tamin D3[25-(OH)D3]and fibroblast growth factor 23(FGF23)in peripheral vascular disease(PVD)in patients with type 2 diabetes mellitus(T2DM).Methods T2DM patients with PVD treated in our hospital from March 2020 to Au-gust 2022 were selected as the pathological group(n=95),T2DM patients without PVD were selected as the non-patho-logical group(n=95),and 95 healthy subjects who underwent physical examination during the same period were select-ed as the control group.The expression levels of serum 25-(OH)D3 and FGF23 were detected by electrochemilumines-cence and ELISA;high-frequency ultrasound was performed among patients with type 2 diabetes,and high-frequency ul-trasound parameters were compared between the diseased group and the non diseased group;ROC curve was applied to analyze the predictive effect of serum 25-(OH)D3 and FGF23 expression levels in type 2 diabetes patients with periph-eral vascular disease,four table method was applied to analyze the diagnostic value of high-frequency ultrasound com-bined with serum 25-(OH)D3 and FGF23 in type 2 diabetes patients with peripheral vascular disease.Results The ex-pression level of serum 25-(OH)D3 in the control group,non lesion group,and lesion group decreased sequentially,while the expression level of serum FGF23 increased sequentially(P<0.05);the AUC of serum 25-(OH)D3 and FGF23 predicting PVD in T2DM patients was 0.743 and 0.766,respectively,and the optimal cutoff values were 24.81μg/L and 54.86 pg/mL,respectively,and the optimal cutoff value was 24.81 g/L and 54.86 pg/mL,respectively;The IMT,plaque area and RI in the pathological group were all higher than those in the non-pathological group,and the PI,the diameter of the dorsal foot artery vessel cavity and PSV were all lower than those in the non-pathological group(P<0.05);the sensitivity of high-frequency ultrasound combined with serum 25-(OH)D3 and FGF23 in the diagnosis of type 2 diabetes patients with peripheral vascular disease was 96.84%,which was higher than that of the three alone(P<0.05).Conclusion Serum 25-(OH)D3 levels decreased and FGF23 levels increased in T2DM patients with PVD.Combined high-frequency ultrasound can better predict the occurrence of PVD in T2DM patients.
