The correlation between gait freezing and anxiety and depression in Parkinson's disease patients

目的 探讨帕金森病患者步态冻结与焦虑抑郁的相关性.方法 选取2020年1月至2022年9月在河北北方学院附属第二医院治疗的60例帕金森病伴步态冻结患者作为观察组,随机选取120例帕金森不伴步态冻结患者作为对照组,比较两组患者临床资料、帕金森病统一评定量表(UPDRS)part Ⅲ评分、汉密尔顿抑郁量表(HAMD)评分、汉密尔顿焦虑量表(HAMA)评分、简易智能精神状态检查量表(MMSE)评分和蒙特利尔认知评估量表(MoCA)评分情况.结果 观察组病程明显长于对照组,H-Y分级明显高于对照组(P<0.05).观察组UPDRS part Ⅲ评分、HAMD评分和HAMA评分明显高于对照组(P<0.05);观察组HAMD评分、HAMA评分与UPDRS part Ⅲ评分呈正相关(r=0.585和0.504,P<0.05).Logistic回归分析结果显示:病程、H-Y分级是帕金森病患者并发步态冻结的影响因素(P<0.05).结论 帕金森病患者步态冻结受病程、H-Y分级的影响,而出现步态冻结时会导致患者焦虑、抑郁等负性情绪,值得进一步研究.
Objective To explore the correlation between gait freezing and anxiety and depression in patients with Parkinson's disease.Methods Sixty Parkinson's disease patients with gait freezing who were treated in our hospital from January 2020 to September 2022 were selected as the observation group,and 120 Parkinson's disease patients without gait freezing were randomly selected as the control group.The clinical data of the two groups of patients,the Unified Parkinson's Disease Rating Scale(UPDRS)Part Ⅲ score,the Hamilton Depression Scale(HAMD)score,and the Hamilton Anxiety Scale(HAMA)score were compared Mini Mental State Examination(MMSE)score and Montre-al Cognitive Assessment(MoCA)score.Results The course of disease in the observation group was significantly lon-ger than that in the control group,and the H-Y grading was significantly higher than that in the control group(P<0.05).The UPDRS part Ⅲ score,HAMD score,and HAMA score in the observation group were significantly higher than those in the control group(P<0.05);The HAMD score,HAMA score,and UPDRS part Ⅲ score in the observa-tion group were positively correlated(r=0.585 and 0.504,P<0.05).Logistic regression analysis showed that the course of disease and H-Y grade were the influencing factors for gait freezing in Parkinson's disease patients(P<0.05).Conclusion Gait freezing in Parkinson's disease patients is influenced by the course of the disease and H-Y grading,and when patients experience gait freezing,it can lead to negative emotions such as anxiety and depression,which is worth further research.
