动态心电图联合淀粉样蛋白A1、miR-499-5p水平在急性心肌梗死早期诊断中的应用价值

目的 探讨动态心电图联合淀粉样蛋白A1(SAA1)、微小RNA(miR)-499-5p水平在急性心肌梗死早期诊断中的应用价值。方法 回顾性分析2021年2月至2023年12月在山西华晋骨科医院接受检查的82例非ST段抬高型急性心肌梗死及82例稳定性心绞痛患者的病历资料。根据患者的疾病类型分为急性心肌梗死组和稳定性心绞痛组,分析急性心肌梗死发病的影响因素,以及动态心电图联合SAA1、miR-499-5p对急性心肌梗死的诊断效能。结果 急性心肌梗死组心肌肌钙蛋白Ⅰ(cTnⅠ)、血浆SAA1、血清miR-499-5p、QTcd高于稳定性心绞痛组(P<0。05)。cTnⅠ(OR=3。728,95%CI:1。489~9。339)、血浆 SAA1(OR=4。076,95%CI:1。627~10。208)、血清 miR-499-5p(OR=3。983,95%CI:1。590~9。976)、QTcd(OR=3。518,95%CI:1。405~8。813)是急性心肌梗死发病的危险因素(P<0。05)。动态心电图联合SAA1、miR-499-5p诊断急性心肌梗死的灵敏度分别为0。73、0。81、0。72、0。92,特异度分别为 0。77、0。79、0。70、0。70,曲线下面积分别为 0。731、0。781、0。735、0。887。结论 血浆 SAA1、血清 miR-499-5p 及QTcd与急性心肌梗死发病有关,三者联合可用于辅助诊断急性心肌梗死。
The Application Value of Dynamic Electrocardiography Combined with Serum Amyloid A1 and miR-499-5p Levels in the Early Diagnosis of Acute Myocardial Infarction
Objective To Explore the Application Value of Dynamic Electrocardiography Combined with Serum Amyloid A1(SAA1)and microRNA(miR)-499-5p Levels in the Early Diagnosis of Acute Myocardial Infarction.Meth-ods A retrospective analysis was conducted on the medical records of 82 patients with non-ST-segment elevation acute myocardial infarction and 82 patients with stable angina who underwent examinations at our hospital from February 2021 to December 2023.Patients were divided into acute myocardial infarction group and stable angina group based on the type of disease.The study analyzed the influencing factors of acute myocardial infarction and the diagnostic efficacy of dynamic electrocardiography combined with SAA1 and miR-499-5p for acute myocardial infarction.Results In the acute myocardial infarction group,levels of cardiac troponin Ⅰ(cTnⅠ),plasma SAA1,serum miR-499-5p,and QTcd were higher compared to the stable angina group(P<0.05).cTnⅠ(OR=3.728,95%CI:1.489~9.339),plasma SAA1(OR=4.076,95%CI:1.627~10.208),serum miR-499-5p(OR=3.983,95%CI:1.590~9.976),and QTcd(OR=3.518,95%CI:1.405~8.813)were identified as risk factors for acute myocardial infarction(P<0.05).The sensitivity of dy-namic electrocardiography combined with SAA1 and miR-499-5p for diagnosing acute myocardial infarction was 0.73,0.81,0.72,and 0.92 respectively,while the specificity was 0.77,0.79,0.70,and 0.70 respectively,and the area under the curve was 0.731,0.781,0.735,and 0.887 respectively.Conclusion Plasma SAA1,serum miR-499-5p,and QTcd are associated with the onset of acute myocardial infarction,and their combination can be used to assist in the diagnosis of acute myocardial infarction.
