Research and Practice of Internet-Based Chronic Disease Management
This paper explores the current state and limitations of traditional chronic disease management models,us-ing these as a starting point to examine the application of internet-based chronic disease management. Key areas in-clude community health services,personalized health services,and the use of mobile terminals,with features such as remote monitoring,smart health devices,online consultations,and personalized health management plans. The integra-tion of internet technology significantly improves the efficiency and convenience of chronic disease management by building a more interactive and responsive management system. This approach not only enhances the quality of health-care services but also effectively reduces healthcare costs,meeting the growing demand for health services. Addition-ally,the system strengthens patient adherence and self-management abilities,thereby improving overall societal health levels. This study also provides a valuable reference for the improvement of public health policies,aiming to further advance chronic disease management systems.
