端端180°旋转吻合术在新生儿肠闭锁中的应用

摘要
目的:先天性小肠闭锁主要的手术方式为肠切除肠吻合术,探讨端端180°旋转肠吻合术在新生儿肠闭锁中的临床应用效果.方法:回顾性分析17例新生儿肠闭锁进行用端端180°旋转肠吻合术的临床资料,并与同时期进行的25例传统的端端吻合术进行对比,分析比较两种不同术式的临床效果.结果:所有患儿术后均进行6～12月随访,端端180°旋转吻合术患儿术后胃肠功能恢复时间、进食时间、住院天数、术后并发症发生率均优于传统端端吻合患儿(P＜0.05).结论:端端180°旋转吻合术操作简单,吻合端两侧血管重建均等,吻合口成角较小,与传统端端吻合术相比其术后胃肠功能术后恢复快,吻合口瘘、肠梗阻等并发症发生率较低,可为临床提供一种新的手术思路.
Abstract
Objective:Congenital intestinal atresia is one of the most common causes of neonatal intestinal obstruction.The purpose of this study was to investigate the clinical effect of end-to-end 180 ° rotation enterostomy in neonatal intestinal atresia.Methods:The clinical data of 17 cases of neonatal intestinal atresia with 180 ° end-to-end rotation enterostomy were retrospec-tively analyzed and compared with 25 cases of traditional end-to-end enterostomy in the same pe-riod.Results:All children were followed up from 6 to 12 months after operation.there was no sig-nificant difference in basic information,atresia site,type and birth weight between end-to-end rota-tional anastomosis group and traditional end-to-end anastomosis group.The postoperative gas-trointestinal function recovery time,feeding time,hospital stay and postoperative complication rate in the 180 ° end-to-end rotational anastomosis group were superior to those in the traditional end-to-end anastomosis group(P＜0.05).Conclusion:End-to-end 180 ° rotational anastomosis is easy to operate,with equal vascular reconstruction on both sides of the anastomotic end and smaller anastomotic Angle.Compared with the traditional end-to-end anastomosis,its postopera-tive gastrointestinal function recovery is faster,the incidence of anastomotic leakage,intestinal ob-struction and other complications is lower,which can provide a new surgical idea for clinicians.
